Pharmacoeconomics of Propofol in Painless Abortion with Quantified Purchasing

目的评价无痛人流术中丙泊酚带量采购药物经济学优势。方法统计某医院2020-11-01—2021-10-31日使用丙泊酚中长链脂肪乳注射液与丙泊酚乳状注射液的宫内早孕患者资料,根据带量采购实施截点(2021-04-30日)和用药分组。采用最小成本分析法分别对带量采购在丙泊酚中长链脂肪乳注射液组和丙泊酚乳状注射液组评价。结果2种丙泊酚在无痛人流中麻醉效果差异无显著性(P>0.05),发生心率减慢、恶心差异无显著性(P>0.05),丙泊酚中长链脂肪乳注射液注射痛发生率低于丙泊酚乳状注射液(P<0.05)。最小成本分析结果表明带量采购品种丙泊酚中长链脂肪乳注射液直接医疗成本(￥2431.54)低于其他各组(￥2861.04、￥2532.68、￥2491.79),与敏感性分析结果一致。结论无痛人流术在取得理想疗效的前提下,带量采购品种丙泊酚中长链脂肪乳注射液更具药物经济学优势。

Objective To evaluate the pharmacoeconomics of propofol in painless abortion with quantified purchasing,providing reference for clinical medication.Methods Data of patients diagnosed as intrauterine pregnancy from November 1st,2020 to October 31st,2021,who were treated with propofol medium and long-chain fat emulsion injection and propofol emulsion injection were collected.Different groups were divided according to implementation time cut-off in quantified purchasing and medication.Each group of propofol medium and long-chain fat emulsion injection and propofol emulsion injection was carried out by the method of cost minimization analysis.Results There was no significant difference in anesthesic effect between the two kinds of propofol in painless abortion(P>0.05).There was no significant difference in the incidence of slowed heart rate and nausea(P>0.05).Propofol medium and long-chain fat emulsion had statistically fewer episodes of injection pain(P<0.05).The cost minimization analysis showed that the direct medical cost of propofol medium and long-chain fat emulsion injection(￥2431.54)was lower than those of other groups(￥2861.04,￥2532.68,￥2491.79),which was consistent with the sensitivity analysis results.Conclusion Propofol medium and long-chain fat emulsion injection has more advantages in pharmacoeconomics on the premise of achieving ideal efficacy in painless abortion.
