缺血性脑卒中患者习得性无助感现状及影响因素分析

Analysis of the status quo and influencing factors of learned helplessness sensation in patients with ischemic stroke

目的观察缺血性脑卒中患者习得性无助感现状,并探讨其影响因素。方法2021年10月—2022年3月,便利抽样法选取在南充市川北医学院附属医院神经内科住院的242例缺血性脑卒中患者为研究对象,采用一般情况调查表、习得性无助感量表、自我感受负担量表对其进行调查。采用t检验或单因素方差分析、Pearson相关分析、多元逐步回归分析缺血性脑卒中患者习得性无助感的影响因素。结果缺血性脑卒中患者习得性无助感总分为(58.54±13.09)分,自我感受负担得分为(31.81±2.28)分。不同居住地、医疗费用支付形式、自理程度、慢性病数量、经济负担、性格、文化程度的缺血性脑卒中患者习得性无助感水平差异均有统计学意义(P均<0.05)。缺血性脑卒中患者习得性无助感与自我感受负担呈正相关(r=0.254,P<0.01)。患者文化程度、性格、自理程度、经济负担、医疗费用支付形式是缺血性脑卒中患者习得性无助感的影响因素(t值依次为-4.391、5.270、3.560、4.499、-4.459,P均<0.05)。结论缺血性脑卒中患者习得性无助感中等偏上,且受患者文化程度、性格、自理程度、经济负担、医疗费用支付形式、自我感受负担的影响。

Objective To observe the status quo of learned helplessness in patients with ischemic stroke and explore its influencing factors.Methods From October 2021 to March 2022,a total of 242 patients with ischemic stroke who were hospitalized in the Department of Neurology,Affiliated Hospital of North Sichuan Medical College of Nanchong were selected as the research objects by convenience sampling.The general information questionnaire,learned helplessness scale and self-perceived burden scale were used to investigate them.T-test or one-way analysis of variance,Pearson correlation analysis and multiple stepwise regression were used to analyze the influencing factors of learned helplessness in patients with ischemic stroke.Results The total score of learned helplessness and self-perceived burden of ischemic stroke patients was(58.54±13.09)and(31.81±2.28)respectively.The results of univariate analysis showed that there were statistically significant differences in the level of learned helplessness among ischemic stroke patients with different places of residence,forms of medical expenses payment,degree of self-care,number of chronic diseases,economic burden,personality and education level(all P<0.05).Pearson correlation analysis showed that the learned helplessness of patients with ischemic stroke was positively correlated with self-perceived burden(r=0.254,P<0.01).The results of multiple linear regression analysis showed that patients’education level,personality,self-care degree,economic burden and form of medical expenses were the influencing factors of learned helplessness in patients with ischemic stroke(t values were-4.391,5.270,3.560,4.499,-4.459,all P<0.05).Conclusion The learned helplessness of patients with ischemic stroke is medium to high,and it is affected by the education level,personality,self-care degree,economic burden,payment form of medical expenses and self-perceived burden.
