组织调整剂在下颌游离缺失可摘局部义齿修复后疼痛处理中的应用

Application of tissue conditioner in treating pain caused by mandibular distal-extension removable partial dentuer

目的:探讨使用临时软衬材料—组织调整剂治疗下颌游离缺失患者可摘局部义齿修复后黏膜损伤性疼痛的临床效果。方法:对5例双端下颌游离缺失可摘局部义齿修复后,因黏膜条件差,多次调改仍然疼痛的患者,使用组织调整剂,以局部衬垫的方式治疗损伤黏膜,同时获得功能状态下的义齿基托组织面形态,然后将基托衬垫部分置换成常规热凝基托树脂,完成义齿修复。通过问卷调查,了解患者在组织调整剂治疗过程中和终义齿戴用后对疼痛、咀嚼、固位效果改善的主观评价。结果:5例患者均获得了有效治疗,黏膜压痛明显缓解,咀嚼功能随之改善。结论:下颌游离缺失可摘局部义齿修复后反复疼痛,用组织调整剂治疗黏膜创伤,再用热凝基托树脂完成义齿的处理方法,可避免软衬材料长期使用的缺陷,最终义齿戴用舒适、功能良好、容易清洁。

Objective:To explore the clinical effects of temporary soft liner—tissue conditioner(TC)in treating recurrent pain caused by mandibular distal-extension removable partial dentuer.Methods:5 patients with bilateral mandibular dissociation loss after removable partial denture repairation were included in this study,they were suffering pain after worn the denture that had even been modified several times.TC was used in these cases to cure traumatized mucosa by local lining,and at the same time,tissue surface morphology of the base plate in functional state was obtained.After the mucosa was cured,TC was replaced with conventional heat-curing resin.Questionnaire survey was conducted to understand patients’subjective evaluation of the improvement of pain,mastication and retention during treatment with TC and after denture wear.Results:All 5 patients received effective treatment,and the mucosa pain was significantly relieved after lining,and mastication function was improved accordingly.Conclusion:For patients with recurrent pain caused by mandibular distal extension removable denture,the mucosa pain was treated with TC first,and then the denture base was completed with hard resin.This treatment method can avoid the defect of soft liner used for a long time,and the final hard resin denture is comfortable to wear,with good function and easy to clean.
