相位角与冠心病PCI患者术前营养状况相关性研究 被引量:2

Correlation between phase angle and preoperative nutritional status in patients with coronary heart disease undergoing PCI

摘要 目的探讨相位角与冠心病经皮冠状动脉介入(PCI)患者术前营养状况相关性。方法收集2021年4-7月在该院心内科拟行PCI手术的120例冠心病住院患者为研究对象。术前采用营养风险筛查表(NRS-2002)、一般情况及膳食情况调查问卷行营养评估,测定人体成分,收集相关营养指标及病情指标,分析相位角与相关指标的相关性。结果相位角与NRS-2002评分、N末端B型利钠肽前体(NT-proBNP)、细胞内外水分比(ECW/TBW)呈负相关(P<0.05),与血红蛋白、BMI、白蛋白、前白蛋白呈正相关(P<0.05)。以NRS-2002评分作为预测患者营养风险标准,相位角在预测患者短期营养风险的特异度为82.5%,灵敏度为93.1%。能量、蛋白质、钾、Vb1、Vb2、镁、钙、锌、硒等摄入不足(P<0.001),脂肪、钠、铁、铜、锰、磷等摄入超量(P<0.001)。结论冠心病PCI术前应用相位角评估其营养状况有助于精准指导该类患者围术期的营养治疗。 Objective To explore the correlation between the phase angle and preoperative nutritional status in the patients with coronary heart disease undergoing PCI.Methods A total of 120 inpatients scheduling to undergo PCI due to coronary heart disease in the cardiology department of this hospital from April to July 2021 were collected as the research subjects.Before surgery, Nutritional Risk Screening(NRS-2002),general information and dietary information questionnaire were used for conducting the nutritional assessment, the body composition was measured, and the relevant nutritional indicators and disease indicators were collected.The correlation between the phase angle and the related indicators was analyzed.Results The phase angle was negatively correlated with the NRS-2002 score, NT-proBNP and ECW/TBW(P<0.05),and positively correlated with Hb, BMI,albumin and prealbumin(P<0.05).With the NRS-2002 score as the standard for predicting the nutritional risk in the patients, the specificity of the phase angle in predicting the short term nutritional risk was 82.5%,and the sensitivity was 93.1%.The intake of energy, protein, potassium, Vb1,Vb2,magnesium, calcium, zinc, selenium, etc.were insufficient(P<0.001),and the intake of fat, sodium, iron, copper, manganese, phosphorus, etc.were excessive(P<0.001).Conclusion Using the phase angle to evaluate the nutritional status is conducive to precisely guide the perioperative nutritional treatment in these patients.
