胰岛素微量泵注在糖尿病酮症酸中毒中的应用效果 被引量:3

摘要 目的探讨胰岛素微量泵注在糖尿病酮症酸中毒中的应用效果。方法选取2019年9月至2021年9月南方医科大学顺德医院的80例糖尿病酮症酸中毒患者作为研究对象进行回顾性分析,采用胰岛素治疗,按照给药方式的不同分成对照组(n=36)和观察组(n=44)。对照组患者采用胰岛素静脉滴注治疗,观察组患者采用胰岛素微量泵注方式。比较两组患者的临床疗效、低血糖发生率、治疗前后血糖指标水平(空腹血糖、餐后2 h血糖)及临床症状缓解时间(血糖恢复时间、pH恢复时间、胰岛素用量、尿酮体转阴时间、血酮体转阴时间)。结果观察组患者治疗后的临床总有效率高于对照组,差异有统计学意义(P<0.05);观察组患者的低血糖发生率低于对照组,差异有统计学意义(P<0.05);观察组患者治疗后的空腹血糖以及餐后2 h血糖均低于对照组,差异有统计学意义(P<0.05);观察组患者的血糖恢复时间、pH恢复时间、尿酮体转阴时间、血酮体转阴时间均短于对照组,胰岛素用量少于对照组,差异有统计学意义(P<0.05)。结论在糖尿病酮症酸中毒患者中应用胰岛素微量泵注方式,对患者临床症状的缓解以及血糖指标的恢复等更具有优势,且治疗有效率更高。 Objective To investigate the application effect of Insulin micropump injection in diabetic ketoacidosis.Methods Eighty patients with diabetic ketoacidosis in Shunde Hospital,Southern Medical University from September 2019 to September 2021 were selected as the study subjects for retrospective analysis.They were treated with Insulin and divided into control group(n=36)and observation group(n=44)according to the administration method.The control group was treated by intravenous infusion of Insulin,and the observation group was treated with Insulin micropump.The clinical efficacy and incidence of hypoglycemia were compared between the two groups,blood glucose index levels(fasting blood glucose,2 h postprandial blood glucose)and clinical symptom relief time(blood glucose recovery time,pH recovery time,insulin dosage,urine ketone body negative time,blood ketone body negative time)were analyzed and compared between the two groups.Results The total clinical effective rate of observation group was higher than that in the control group after treatment,the difference was statistically significant(P<0.05).The incidence of hypoglycemia in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).The fasting blood glucose and 2 h postprandial blood glucose in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The blood glucose recovery time,pH recovery time,urine ketone body negative time and blood ketone body negative time in observation group were shorter than those in control group,insulin dosage in observation group was less than that in control group,the differences were statistically significant(P<0.05).Conclusion In patients with diabetic ketoacidosis,the application of Insulin micropump has more advantages in the remission of clinical symptoms and the recovery of blood glucose indicators,and the treatment efficiency is higher.
