电刺激结合牵张训练在盆底肌张力增高患者诊疗中的临床应用

摘要 目的研究电刺激生物反馈结合牵张训练在盆底肌张力增高患者诊疗中的应用效果。方法回顾性选取2021年1月至12月宜春市妇幼保健院收治的100例盆底肌张力增高患者作为研究对象,依据治疗方式不同分为对照组(n=50)与观察组(n=50),对照组采取牵张训练,观察组采用生物刺激反馈仪给予肌肉电刺激结合牵张训练,比较两组患者治疗前后盆底肌力生理指标(前、后静息平均肌电值)、盆底肌功能障碍问卷(PFDI-20)及视觉模拟评分法(VAS)得分。结果治疗前,两组患者PFDI-20、VAS得分及盆底肌前、后静息平均肌电值比较,差异均无统计学意义(P>0.05);治疗后,观察组PFDI-20得分高于对照组,VAS得分,盆底肌前、后静息平均肌电值均低于对照组,差异有统计学意义(P<0.05)。结论电刺激结合牵张训练可明显改善盆底肌张力增高患者的盆底肌肌力及盆底肌肌电位,有效改善其临床症状,缓解其疼痛感,可在临床推广使用。 Objective To study the application effect of electrical stimulation combined with stretching exercise in the diagnosis and treatment of patients with increased pelvic floor muscle tone.Methods A total of 100 patients with increased pelvic floor muscle tone treated in Yichun Maternal and Child Health Hospital from January to December 2021 were retrospectively selected as research objects,they were divided into control group(n=50)and observation group(n=50)according to different treatment methods.The control group was given stretching exercise,and the observation group was given muscle electrical stimulation using the biological stimulation feedback instrument combined with stretching exercise.The physiological indicators of pelvic floor muscle strength(average electromyography value of pre-resting and post-resting),the pelvic floor distress inventory-short form 20(PFDI-20)score and the visual analogue scale(VAS)score before and after treatment were compared between two groups.Results There were no statistically significant differences in PFDI-20 score,VAS score or average electromyography value of pre-resting and post-resting between the two groups before treatment(P>0.05).After treatment,the PFDI-20 score of observation group was higher than that of control group,the VAS score and average electromyography value of pre-resting and post-resting of observation group were lower than those in control group,the differences were statistically significant(P<0.05).Conclusion Electrical stimulation combined with stretching exercise can significantly improve pelvic floor muscle strength and pelvic floor muscle potential,effectively improve clinical symptoms,and relieve pain in patients with increased pelvic floor muscle tone.
