摘要
目的探究极速脉搏波技术(UFPWV)测量早期2型糖尿病患者颈动脉内膜弹性,并以此评价早期动脉硬化的应用价值。方法选择2019年8月—2021年8月本院收治的2型糖尿病(T2DM)患者80例作为研究对象,根据颈动脉内中膜厚度分为T2DM 1组(43例)和T2DM 2组(37例),另外选择50名同期健康志愿者作为对照组。收集研究对象基线数据,采用UFPWV测定所有研究对象颈动脉内中膜厚度、PWVBS和PWVES,独立样本t检验分析组间PWVES和PWVBS相关性,多元逐步回归分析T2DM患病因素与PWVES以及PWVBS的关系。结果对照组、T2DM 1组和T2DM 2组三组组间左侧颈动脉PWVBS和PWVES均略高于右侧测量值,但差异无统计学意义(P>0.05),其中T2DM 2组的PWVBS和PWVES值大于T2DM 1组(P<0.05);T2DM组PWVBS和PWVES值大于对照组(P<0.05);多元逐步回归分析结果显示,年龄、舒张压、体重指数和总胆固醇等因素影响PWVBS和PWVES值。结论UFPWV技术可以快速测量2型糖尿病患者颈动脉内膜IMT、PWVBS和PWVES,具有评价测量早期2型糖尿病患者颈动脉内膜弹性在评价早期动脉硬化的价值。
Objective To measure elasticity of carotid artery intima in patients with early type 2 diabetes by using ultra-fast pulse wave velocity(UFPWV)technology,in order to evaluate its application value in assessing early arteriosclerosis.Methods A total of 80 patients with type 2 diabetes(T2DM)diagnosed in our hospital during August 2019 and August 2021 were collected as study subjects,and they were divided into T2DM1 group and T2DM 2 group according to the thickness of the carotid artery intima-media.In addition,50 healthy volunteers were selected as the control group.The baseline data of the study subjects were collected,UFPWV was use to determine the carotid artery intima-media thickness,PWVBS and PWVES.Independent sample t-test was used to analyze the correlation between PWVES and PWVBS,and multi-step stepwise regression analysis was performed to analyze the relationship between T2DM ill factors and PWVES and PWVBS.Results The PWVBS and PWVES of the left carotid artery in the three groups of the control group,T2DM 1 and T2DM 2 group were slightly higher than the measured values on the right side,but there was no statistical difference(P>0.05).Among them,the PWVBS and PWVES values of the T2DM 2 group were greater than those of the T2DM 1 group(P<0.05).The PWVBS and PWVES values of the T2DM group were greater than those of the control group(P<0.05).The results of multiple stepwise regression analysis showed that age,diastolic blood pressure,and body mass index and total cholesterol affect the values of PWVBS and PWVES.Conclusions UFPWV technology could quickly detect the IMT,PWVBS and PWVES of carotid artery intima in patients with type 2 diabetes,and it has important value for evaluating the carotid artery intima elasticity in patients with early type 2 diabetes.
