梅花针叩刺与常规针刺治疗肝胃不和型功能性消化不良的随机对照研究

Randomized controlled study of plum blossom acupuncture and traditional acupuncture for the treatment of functional dyspepsia with discordant liverand stomach

目的:基于“双固一通”理论,观察梅花针叩刺与常规针刺治疗肝胃不和型功能性消化不良(FD)的临床疗效。方法:选择120例肝胃不和型FD患者随机分为对照组、观察组1、观察组2,每组40例。对照组予以口服奥美拉唑肠溶胶囊联合克拉霉素缓释片、阿莫西林胶囊,观察组1行梅花针叩刺关元穴及双侧足三里、太冲穴,观察组2行常规针刺,穴位同前,比较各组患者临床症状、生活质量和检查指标的改变。结果:与对照组比较,干预后,观察组1、观察组2总有效率、胃排空率、SF-36评分、NDI评分均显著升高(P<0.05),证候评分显著降低(P<0.05),观察组1与观察组2 SF-36评分、NDI评分及证候评分治疗前后差值均显著高于对照组(P<0.05),且观察组1反酸症状评分治疗前后差值显著高于观察组2(P<0.05)。结论:梅花针叩刺与常规针刺对肝胃不和型FD均具有较高的临床医疗价值,梅花针在改善反酸症状上有其独特的优势。

Objective:Based on the theory of‘double-reinforcing and one-unblocking',the clinical efficacy of plum blossom acupuncture and traditional acupuncture in the treatment of functional dyspepsia(FD)with liver and stomach disharmony was observed.Methods:One hundred and twenty patients with FD of hepatogastric disharmony were selected and randomly divided into control group,observation group 1 and observation group 2,with 40 cases in each group.The control group was given oral omeprazole enteric dissolution capsule combined with clarithromycin extended-release tablets and amoxicillin capsule,and the observation group 1 was given plum blossom acupuncture by tapping Guanyuan and bilateral Zusanli and Taichong.The observation group 2 was given traditional acupuncture with the same acupoints as before.The clinical symptoms,quality of life and examination indexes of patients in each group were compared.Results:Compared with the control group,the total effective rate,gastric emptying rate,SF-36 score and NDI score of observation group 1 and observation group 2 were significantly higher(P<0.05),and the evidence score was significantly lower after the intervention(P<0.05).The difference between the SF-36 score,NDI score and symptom score of observation group 1 and observation group 2 before and after treatment was greater than that of the control group(P<0.05).The difference of acid regurgitation score before and after treatment in observation group 1 was greater than that in observation group 2(P<0.05).Conclusion:Both plum blossom acupuncture and traditional acupuncture have high clinical medical value for FD with liver and stomach disharmony.Plum blossom acupuncture has its unique advantages in improving acid reflux symptoms.
