摘要
目的:探讨前列腺影像报告和数据系统2.1版(PI-RADS v2.1)中DWI评分和ADC值与前列腺癌Gleason分级分组(GGG)的相关性。方法:回顾性分析经病理证实的32例前列腺良性病变(良性病变组)和94例前列腺癌(前列腺癌组)患者。2组均行常规MRI序列(T1WI、T2WI)、DWI(b=0、800 s/mm^(2))序列扫描。根据病理结果,将前列腺癌GGG分为5组。DWI评分与GGG之间一致性行Kappa检验。ADC值与GGG的相关性行秩相关统计。2组ADC值比较行两独立样本t检验。结果:b=800 s/mm^(2)时良性病变、前列腺癌组的ADC值分别为(1.198±0.147)×10^(-3) mm^(2)/s、(0.829±0.088)×10^(-3) mm^(2)/s,差异有统计学意义(t=12.355,P<0.05)。94例前列腺癌的ADC值与GGG呈负相关(rs=-0.935,P<0.05),PI-RADS v2.1中的DWI评分与GGG具有较好的一致性(K=0.717,P<0.001)。结论:PI-RADS v2.1中DWI评分和ADC值与前列腺癌GGG相关,可在术前准确评估前列腺癌。
Objective:To investigate the correlation between the DWI score of prostate imaging reporting and data system version 2.1(PI-RADS v2.1)and ADC values and Gleason Grade Groups(GGG)of prostate cancer.Methods:The clinical data of 94 patients with prostate cancer(the malignant group)and 32 cases with benign prostate lesions(the benign group)were respectively analyzed.All patients underwent T1WI,T2WI,DWI(b=0,800 s/mm^(2))sequences.According to the pathologic results,GGG of prostate cancer had 5 groups.Kappa test was performed for the consistency between DWI score and GGG.Rank correlation was performed for correlation between ADC value and GGG.Two-sample independent t test was performed for comparison of ADC values between the two groups.Results:When b value took 800 s/mm^(2),the ADC value of the benign group and the malignant group was(1.198±0.147)×10^(-3) mm^(2)/s and(0.829±0.088)×10^(-3) mm^(2)/s,respectively,with a significant difference(t=12.355,P<0.05).The ADC value of the malignant group was negatively correlated with GGG(rs=-0.935,P<0.05),and the DWI score of PI-RADS v2.1 and GGG had a good consistency in the malignant group(K=0.717,P<0.001).Conclusions:The DWI score of PI-RADS v2.1 and ADC values are related to the GGG of prostate cancer,which has an important diagnostic value in accurately assessing prostate cancer before surgery.
