摘要
目的 分析辅助性T细胞17(Th17)/调节性T细胞(Treg)比例失衡与肠道病毒71型(EV71)感染手足口病患儿病情程度的关系及临床意义。方法 将2019年8月—2021年12月上海健康医学院附属嘉定区中心医院收治的EV71感染手足口病患儿150例纳入研究,EV71感染手足口病患儿根据病情严重程度分为普通型组125例,重型组25例,另择同期90例体检健康儿童作为对照组,回顾性收集研究对象的临床资料。比较3组全血Th17、Treg、Th17/Treg水平,血清Th17、Treg相关细胞因子水平,通过Pearson相关分析法分析全血Th17、Treg、Th17/Treg与疾病严重程度的相关性。结果 重型组、普通型组、对照组全血Th17、Th17/Treg水平呈下降趋势,全血Treg水平呈升高趋势,组间比较差异有统计学意义(P<0.05)。重型组、普通型组、对照组白细胞介素-17(IL-17)、IL-6水平呈下降趋势,血清IL-23、IL-10、转化生长因子-β(TGF-β)水平呈升高趋势,组间比较差异有统计学意义(P<0.05)。Pearson相关分析法结果显示,疾病严重程度与全血Treg水平呈负相关(r=-0.430,P<0.05),疾病严重程度与全血Th17、Th17/Treg水平呈正相关(r=0.713、0.352,P<0.05)。结论 EV71感染手足口病患儿感染后全血Th17水平提高,全血Treg降低,Th17/Treg比例失衡,可加重机体炎症反应,与病情严重程度密切相关。
Objective To analyze the relationship between the imbalance of helper T cell 17(Th17)/regulatory T cell(Treg) ratio and the severity of children with enterovirus 71(EV71) infected hand, foot and mouth disease and its clinical significance. Methods A total of 150 children with EV71 infected hand, foot and mouth disease admitted to Jiading District Central Hospital Affiliated to Shanghai University of Medicine & Health Sciences between August 2019 and December 2021 were enrolled in the study. According to the severity of the disease, the children with EV71 infected hand, foot and mouth disease were divided into the common group(n=125) and severe group(n=25), and 90 healthy children in the same period were selected as the control group. The clinical data of the subjects were collected retrospectively. The levels of Th17, Treg and Th17/Treg in whole blood and serum Th17 and Treg-related cytokine levels among the three groups were compared, and the correlation between the levels of Th17, Treg and Th17/Treg in whole blood and the severity of the disease was analyzed by Pearson correlation analysis. Results The levels of Th17 and Th17/Treg in the whole blood of severe, common and control groups showed a decreasing trend, while the levels of Treg in whole blood showed an increasing trend. There was statistically significant difference between the groups(P<0.05). The levels of serum interleukin-17(IL-17) and interleukin-6(IL-6) in the severe, common and control groups showed a decreasing trend, while the levels of serum interleukin-23(IL-23), interleukin-10(IL-10) and transforming growth factor-β(TGF-β) showed an increasing trend. There was statistically significant difference between the groups(P<0.05). The results of Pearson correlation analysis showed that the severity of the disease was negatively correlated with the levels of Treg in whole blood(r=-0.430, P<0.05) and positively correlated with the levels of Th17 and Th17/Treg in whole blood(r=0.713 and 0.352, P<0.05). Conclusion The children with EV71 infected hand, foot and mouth disease have increased levels of Th17, decreased levels of Treg and unbalanced Th17/Treg in whole blood, which may aggravate the inflammatory response of the body and is closely related to the severity of the disease.
