矾藤痔硬化剂注射治疗Ⅰ~Ⅱ度内痔的临床疗效观察

Clinical effect of Fanteng sclerosing agent for hemorrhoids in the treatment of gradeⅠtoⅡinternal hemorrhoids

目的观察矾藤痔硬化剂注射治疗Ⅰ~Ⅱ度内痔的临床疗效。方法纳入2020年1月至2020年12月于河北省中医院肛肠科就诊的70例Ⅰ~Ⅱ度内痔患者作为研究对象,按照随机数字表法分为观察组(予矾藤痔硬化剂注射治疗,n=35)和对照组(予消痔灵硬化剂注射治疗,n=35)。比较两组临床疗效,症状评分(痔核大小、便血、痔核脱出),术后并发症发生情况(肛门坠胀、肛门疼痛、痔核黏膜充血糜烂、直肠硬结),HEMO-FISS-QoL量表评分。结果两组临床疗效及痊愈率比较差异无统计学意义(均P>0.05)。两组术前痔核大小、便血、痔核脱出评分比较差异均无统计学意义(均P>0.05);两组术后14 d痔核大小、便血、痔核脱出评分均低于术前(均P<0.05),但两组间比较差异均无统计学意义(均P>0.05)。观察组术后1 d肛门坠胀、肛门疼痛发生率及术后14 d痔核黏膜充血糜烂、直肠硬结发生率均低于对照组(均P<0.05)。两组术前HEMO-FISS-QoL量表评分比较差异无统计学意义(P>0.05);两组术后60 d HEMO-FISS-QoL量表评分均较术前降低,且观察组HEMO-FISS-QoL量表评分低于对照组(均P<0.05)。结论矾藤痔硬化剂注射治疗Ⅰ~Ⅱ度内痔具有良好的临床疗效,能够改善患者便血、痔核脱出等症状,提高患者生活质量,值得临床推广应用。

Objectives To investigate the clinical effect of Fanteng sclerosing agent for hemorrhoids for treating gradeⅠtoⅡinternal hemorrhoids.Methods Seventy patients with gradeⅠtoⅡinternal hemorrhoids at the Department of Proctology,Hebei Hospital of Traditional Chinese Medicine,were recruited between January and December 2020.Patients were randomly assigned to the treatment group(receiving infection of Fanteng sclerosing agent for hemorrhoids,n=35)and the control group(receiving injection of Xiaozhiling sclerosing agent,n=35)using a random number table.The followings were compared between the two groups:effectiveness,symptom scores(size of hemorrhoids,hematochezia,and prolapsed hemorrhoids),postoperative complications(anal distension,anal pain,congestion and erosion of hemorrhoids mucous,and rectal hard nodule),and HEMO-FISS-QoL questionnaire score.Results The proportions of effectiveness and cure rate were comparable between the two groups(P>0.05).Before surgery,the two groups did not differ significantly in terms of scores of the size of hemorrhoids,hematochezia,and prolapsed hemorrhoids(P>0.05).At 14 days after surgery,both groups showed significantly lower scores of the size of hemorrhoids,hematochezia,and prolapsed hemorrhoids than before surgery(P<0.05),while these scores at 14 days after surgery were comparable between the two groups(P>0.05).The incidence of anal distension and pain 1 day after surgery and the incidence of congestion and erosion of hemorrhoids mucous and rectal hard nodule 14 days after surgery were significantly lower in the treatment group than in the control group(P<0.05).Preoperative HEMO-FISS-QoL score was comparable between the two groups;HEMOFISS-QoL scores at 60 days after surgery decreased from baseline in both groups,with significantly lower score in the treatment group than in the control group(P<0.05).Conclusion Injection of Fanteng sclerosing agent for hemorrhoids is effective for treating gradeⅠtoⅡinternal hemorrhoids.It can improve patients’symptoms,such as hematochezia and prolapsed hemorrhoids,improve patients’quality of life,and is worthy of clinical promotion and application.
