CBL教学联合肾脏专科操作培训在专科医师规范化培训中应用研究

摘要 目的探讨CBL教学联合肾脏专科操作培训在专科医师规范化培训中应用研究的效果。方法研究对象为2020年12月1日-2021年5月31日某院住院医师规范化培训的肾脏专科研究生30名,随机分为观察组15例和对照组15例,对照组采用传统带教模式,观察组采用CBL教学联合专科操作技能培训。比较2组临床诊疗思维考核、临床操作技能考核及满意度。结果观察组临床病例诊疗分析考核成绩为(94.35±1.04)分,高于对照组的(90.16±1.50),P<0.05,差异具有统计学意义。观察组CVC置入术、TCC置入术、AVF成形术、腹透导管植入术经过指导老师按照临床技能考核细则进行考核,观察组成绩分别(95.50±2.50)分、(92.32±3.04)分、(93.15±2.35)分和(90.45±3.05)分,高于对照组的(89.45±4.15)分、(86.05±2.34)分、(87.24±3.15)分和(85.50±2.32)分,组间比较,P<0.05,差异有统计学意义。观察组分别对临床思维能力培训、临床操作技能培训、临床执业胜任力信心度提升及总体满意度均高于对照组,差异有统计学意义,P<0.05。结论CBL教学联合肾脏专科操作培训在中医肾脏专科医师规范化培训能够培养出合格的初级肾脏专科医师,但仍需不断总结改进及优化培训模式。 Objective To explore the effect of CBL teaching combined with operation training of renal specialist in the standardized training of specialists.Methods The research object was Graduate Students in Nephrology in standardized training of resident doctors in a hospital from December 1st,2020 to May 31st,2021,who were randomly divided in the observation group with 15 cases and the control group with 15 cases.The control group adopted the traditional teaching mode,and the observation group adopted the CBL teaching combined with specialized operation skills training.Clinical diagnosis and treatment thinking assessment,clinical operation skills assessment and satisfaction of the cases from the two groups were compared.Results Compared with the control group,the scores of the disease case diagnosis and treatment analysis and assessment in the observation group were higher,and there were significant differences between these two groups(P<0.05).All the assessment scores of central venous catheter(CVC)implantation operation,TCC implantation operation,Arteriovenous fistula(AVF)plasty operation,abdominal catheterization and other examinations in the observation group were higher than those of the control group,and there were significant differences between these two groups(P<0.05).Clinical thinking ability training,clinical operation skills training,improved confidence and overall satisfaction of clinical practice competence in the observation group were higher than those of the control group,and there were significant differences between these two groups(P<0.05).Conclusions CBL combined with kidney specialty operation training in the standardized training of Traditional Chinese Medicine(TCM)kidney specialists could train qualified primary kidney specialists,but it still needed to constantly summarize and improve and optimize the training mode.
