刘小虹辨治咳嗽晕厥综合征经验探析

Analysis of LIU Xiaohong′s Experience in Differentiating and Treating Cough Syncope Syndrome

咳嗽晕厥综合征是指在连续、剧烈咳嗽的情况下出现一过性的意识丧失,不宜留任何后遗症,但其发病机制尚未完全明确,治疗上主要针对原发疾病,辅以镇咳药物,并加强日常调护。在中医学上,咳嗽晕厥综合征归属于“厥证”“风咳”范畴,刘小虹教授认为气机逆乱是本病的基本病机,外邪侵袭则为发病的诱因。风邪为首,侵袭肺卫,引动内在痰邪,痰气阻于气道,肺失清肃,肺气上逆而作咳;其病位在肺,又不离肝脾、胃,肝肺气机失调、脾胃气机升降相因失职而引发的气机逆乱是咳嗽性晕厥的发病关键。在治疗上刘小虹教授强调疏风宣肺以散邪,理肺气之时同调肝脾,止咳化痰贯穿疾病始终,并在日常生活注意调护。处方用药上刘教授重视临床辨证,在发扬岭南医家临床实践经验的基础上总结而出的三叶汤,用药轻灵,恰和肺性,在治疗咳嗽晕厥综合征上取得了满意的效果。

Cough syncope syndrome refers to a transient loss of consciousness in the case of continuous and severe cough,and it is not appropriate to leave any sequelae.However,its pathogenesis is not completely clear.The treatment is mainly aimed at the primary disease,supplemented by antitussive drugs,and daily care is strengthened.In Traditional Chinese Medicine,cough syncope syndrome is classified as“syncope”or“wind cough”.According to Professor LIU Xiaohong,Qi disturbance is the basic pathogenesis of the disease,and the invasion of external pathogens is the inducement.The wind evil is the leading factor,attacking the lung and Wei phase,inducing the internal phlegm eviland the phlegm Qi blocks the airway.The lung loses its clearing function,and the lung Qi goes upward reversely,leading cough.The disease is located in the lung,as well as in the liver,spleen and stomach.The disorder of liver and lung Qi movement and the ascending and descending Qi of spleen and stomachis the key to the occurrence of cough syncope.In treatment,Professor LIU emphasizes on dispelling wind and promoting lung to dissipate evil,coordinating liver and spleen when regulating lung Qi,relieving cough and reducing phlegm throughout the disease,and paying attention to nursing in daily life.Professor LIU attaches great importance to clinical syndrome differentiation and Sanye Decoction(三叶汤),which was summarized on the basis of developing the clinical practice experience of Lingnan doctors.The effect of Sanye Decoctionis quite mild,suitable for the lungand has satisfactory therapeutic effects for cough syncope syndrome.
