姜树民治疗慢性萎缩性胃炎临证经验探赜

Exploration of JIANG Shumin′s Clinical Experience on Treating Chronic Atrophic Gastritis

慢性萎缩性胃炎属于临床中常见且难治性疾病,而中医治疗慢性萎缩性胃炎在改善患者临床症状,延缓疾病进展,疏导患者焦虑情绪方面具有较大优势。该文从辨证思路、治疗特色、临证施治等方面系统阐述姜树民教授治疗慢性萎缩性胃炎的学术思想与经验。辨证思路上,提出脾胃虚弱,胃失和降为本,湿热留恋为标,二者互为因果、相互促进。并引入“以痈论治”思想,将中医外治法的思路融入本病的治疗,亦起斡旋中州之妙。治疗特色上,以“消、托、补”三法,扶正固本,祛邪外出,顾护中焦,衷中参西,于无形中疏导患者焦虑情绪。临证施治上,承前人之学术思想,立足整体,谨守病机,方用“消痈建中汤”,临证化裁,改善慢性萎缩性胃炎临床症状,为临床诊治提供参考和思路。

Chronic atrophic gastritis is a common and refractory disease in clinic,and TCM treatment of chronic atrophic gastritis has greater advantages in improving patients′clinical symptoms,delaying disease progression and relieving patients′anxiety.This article systematically described the academic thought and experience of Professor JIANG Shumin in the treatment of chronic atrophic gastritis in terms of the identification of evidence,the treatment of clinical evidence and the analysis of experimental cases.The idea of syndrome differentiation considers deficiency of spleen and stomach and stomach failing to harmonize as the root,and the retention of dampness and heat as the symptoms,interacting as both cause and effect.He also introduced the idea of“syndrome differentiation according to carbuncle”and incorporated the idea of external traditional Chinese medicine into the treatment of this disease,which is also a good way to mediate the two states.The treatment is characterized by the three methods of“eliminating,supporting and tonifying”,which help to consolidate the root,dispel the evil,protect the middle energizer,and take into account the Chinese and Western aspects to ease the patients′anxiety in an invisible way.In the clinical treatment,we inherited the academic thinking of our predecessors,based on the whole body and adhering to the disease mechanism,and used“Xiaoyong Jianzhong Decoction(消痈建中汤)”to improve the clinical symptoms of chronic atrophic gastritis,providing reference and ideas for clinical diagnosis and treatment.
