复方苦参注射液联合同步放化疗治疗局部晚期食管癌的临床研究

Clinical research of locally advanced esophageal cancer patients treated Compound kushen injection combined with concurrent radiochemotherapy

目的 研究复方苦参注射液联合同步化疗放疗治疗局部晚期食管癌的疗效。方法 将68例局部晚期食管癌随机分为观察组和对照组各34例,对照组患者只接受同步放化疗,观察组在放化疗的基础上给予注射复方苦参注射液,从放化疗开始连续用至治疗结束,治疗后评价疗效和毒副反应。结果 对照组总有效率(CR+PR)为58.82%;观察组总有效率(CR+PR)为70.59%,两组间总有效率差异无统计学意义(P>0.05)。治疗后患者睡眠、社交、正常工作、行走能力和情绪5项生活质量评分比较,差异具有统计学意义(P<0.05)。观察组大于2级的放射性食管炎、放射性肺炎及骨髓抑制等毒副反应总发生率为14.7%,远低于对照组的47.06%,两组间差异有统计学意义(P<0.05)。结论 复方苦参注射液同步联合放化疗有提高生活质量及临床疗效,对放化疗的毒副反应有减轻作用。

Objective The effect of Compound kushen injection combined with synchronous chemotherapy and radiotherapy in the treatment of locally advanced esophageal cancer was studied.Methods The 68 cases of local advanced esophageal cancer were randomly divided into the observation group and 34 patients in the control group.The control patients only received synchronous radiotherapy and chemotherapy.The observation group gave the injection of Compound kushen injection on the basis of chemoradiotherapy, with continuous use from radiotherapy and chemotherapy to the end of treatment, and the efficacy and toxic reactions were evaluated after treatment.Results The total effective rate(CR+PR)of patients in the observation group was superior to that patients in The concurrent radiochemotherapy alone(70.59% vs 58.82%,P>0.05).All 5 assessments of QOL,including sleep, social,work,ambulation and emotion life in both groups were improved after treatment(P<0.05).Also the total incidence rate of more than Ⅱ levels of radiation esophageal toxicity, radiation pneumonitis and bone marrow supression toxicity were much higher in the control group than the observation group(47.06% vs 14.7%,P<0.05).Conclusion The synchronous combination of Compound kushen injection and chemoradiotherapy and chemotherapy can improve the quality of life and clinical efficacy, and has a reducing effect on the toxic response of chemoradiotherapy and chemotherapy.
