VMI与MSCT对鉴别肺磨玻璃结节良恶性的价值

Value of VMI and MSCT in differentiating benign and malignant pulmonary ground glass nodules

目的分析VMI与MSCT对肺磨玻璃结节良恶性的临床鉴别价值。方法选取郑州市第十五人民医院2020年10月—2021年10收治的81例肺磨玻璃结节患者为研究对象,所有患者均给予VMI、MSCT检查,分析患者所得图像进行并详细记录结果,以穿刺活检或手术病理结果为“金标准”分析VMI、MSCT对肺磨玻璃结节良恶性的临床鉴别价值。结果81例肺磨玻璃结节患者,良性39例、恶性42例,其中单纯型肺磨玻璃结节33例(良性23例、恶性10例),混合型肺磨玻璃结节48例(良性16例、恶性32例),2种诊断方式对肺磨玻璃结节良性诊断符合率存在差异性(P<0.05),且相比VMI,MSCT影像灵敏度、准确度、AUC值较高,特异度较低,差异具有统计学意义(P<0.05)。结论VMI与MSCT对肺磨玻璃结节良恶性鉴别均具有良好的临床诊断价值,其中MSCT在病灶检出及肺磨玻璃结节定性诊断中准确度更高,可更为准确的鉴别肺磨玻璃结节良恶性。

Objective To analyze the clinical value of VMI and MSCT in differentiating benign and malignant pulmonary ground glass nodules.Methods A total of 81 patients with pulmonary ground glass nodules who were admitted to our hospital from October 2020 to October 2021 were selected as the research objects.All patients were given VMI and MSCT examination,and the images obtained from the patients were analyzed and the results were recorded in detail.The value of VMI and MSCT in differentiating benign and malignant pulmonary ground glass nodules was analyzed using biopsy or surgical pathological results as the“gold standard”.Results Totally 81 patients with pulmonary grinding glass nodules,42 cases of malignacy,39 cases of benign,and nodules with simplex pulmonary grinding glass 33 cases(23 cases of benign and 10 cases of malignacy),mixed lung grinding glass nodules 48 cases(16 cases of benign and 32 cases of malignant),two methods for diagnosis of pulmonary grinding glass nodules benign diagnosis coincidence rate difference(P<0.05),and compared with the VMI,The sensitivity,accuracy and AUC values of MSCT images were higher,but the specificity was lower,and the difference was statistically significant(P<0.05).Conclusion Both VMI and MSCT have good clinical diagnostic value in differentiating benign and malignant pulmonary ground glass nodules.MSCT has higher accuracy in lesion detection and qualitative diagnosis of pulmonary ground glass nodules,which can more accurately distinguish benign and malignant pulmonary ground glass nodules.
