Establishment and application of evaluation criteria for drug utilization of recombinant human erythropoietin

目的建立重组人促红细胞生成素(rHuEPO)药物利用评价标准,分析龙岩市三家医院rHuEPO合理应用情况,为临床合理用药提供参考。方法以rHuEPO药品说明书为基础,参考相关指南和文献,采用专家咨询法,与临床和院外专家讨论,建立rHuEPO药物利用评价标准。采用回顾性调查方法,随机抽取龙岩市三家医院2021年使用rHuEPO的出院患者病例进行合理性评价。结果三家医院共纳入422例病例,其中A院213例、B院104例、C院105例。存在主要问题为用法用量不合理31.75%(134/422)、用药疗程不规范28.44%(120/422)、CKD患者用药前未行铁参数检测54.72%(168/307)、规律透析CKD患者用药未每月行铁参数检测76.53%(150/196)。结论rHuEPO药物利用评价标准有较强的科学性、实用性和可操作性,可发现rHuEPO临床应用过程中的问题,药学部门应积极干预,促进其合理用药。

Objective To establish the evaluation criteria of recombinant human erythropoietin(rHuEPO)drug utilization and analyze the rational application of rHuEPO in three hospitals in Longyan City,so as to provide reference for clinical rational drug use.Methods The evaluation criteria for rHuEPO drug utilization were established based on the drug label of rHuEPO,with reference to relevant guidelines and literature,and the expert consultation method was used to discuss with clinical and outside experts.A retrospective investigation was conducted to randomly select discharged patients who used rHuEPO in 2021 from three hospitals in Longyan City for rationality evaluation.Results A total of 422 cases were included in the three hospitals,including 213 cases in hospital A,104 cases in hospital B and 105 cases in hospital C.The main problems were unreasonable usage and dosage 31.75%(134/422),irregular medication course 28.44%(120/422),iron parameters were not detected in CKD patients before medication 54.72%(168/307),and iron parameters were not detected in regular dialysis CKD patients every month 76.53%(150/196).Conclusion The evaluation criteria of rHuEPO drug utilization are scientific,practical and operable,which can find the problems in the clinical application of rHuEPO,pharmaceutical departments should actively intervene to promote its rational drug use.
