精液温度对同源卵母细胞妊娠结局的影响

摘要 目的 探讨体外受精(IVF)短时受精时精液优化处理后到授精前放置温度对同源卵母细胞受精和妊娠结局的影响。方法 取卵当日,其丈夫用手淫法取精,液化后采用密度梯度离心联合上游法(DGC+CSW)进行精液优化处理,将精液分为两管,一管放置于室温,一管放置于37℃培养箱。将获卵数≥10个的IVF短时受精助孕患者的同源卵子随机分为两组,分别用室温(室温组)和培养箱(对照组)精液行IVF短时受精,比较两组受精及胚胎发育情况。结果 本研究共纳入180个短时受精周期,其中IVF正常受精161个周期,补救单精子卵胞质显微注射技术(Re-ICSI)19个周期。室温组和对照组MⅡ卵数和MⅡ卵率比较,差异无统计学意义(P>0.05);室温组IVF正常受精周期和Re-ICSI周期的受精率、卵裂率、优质胚胎率和囊胚率均稍高于对照组,但差异无统计学意义(P>0.05)。结论 IVF短时受精前优化处理后精液放置温度对妊娠结局无明显影响。 Objective To investigate the effect of semen temperature from optimal treatment to pre-insemination on homologous oocyte fertilization and pregnancy outcome during short term fertilization in vitro fertilization(IVF). Methods On the day of oocytes retrieval, male took sperm by masturbation. The semen was separated by density gradient centrifugation combined with upstream method(DGC+CSW) after liquefaction. The sperm was divided into two tubes, one tube was placed at room temperature, and the other was placed at 37℃ incubator. Homologous ovum of patients with ≥10 ovum harvested by IVF short-time fertilization to assist pregnancy were randomly divided into two groups, respectively using room temperature(room temperature group) and incubator(control group) semen for short-time fertilization in IVF, and the fertilization and embryonic development of the two groups were compared. Results A total of 180 short-time fertilization cycles were included in this study, in which 161 IVF cycles and 19 rescure-intracytoplasmic sperm injection(Re-ICSI) cycles. There was no significant difference in the number of MⅡ oocytes and MⅡ oocytes rate between room temperature group and the control group(P>0.05). The fertilization rate, cleavage rate, high-quality embryo rate and blastocyst rate were slightly higher in the room temperature group compared with the control group, but the difference was not statistically significant(P>0.05). Conclusions The temperature of sperm before IVF short-term fertilization has no significant effect on the pregnancy outcome.
