左心房左外侧脊长度与房颤患者射频消融术后房颤复发的关系

Relationship between left lateral ridge length of left atrium and atrial fibrillation recurrence after radiofrequency ablation in patients with atrial fibrillation

目的 探讨左心房左外侧脊(LLR)长度与心房颤动(房颤)射频消融术后房颤复发的关系。方法 房颤患者80例进行左心房CT三维重建,根据左心房中位LLR长度分为短LLR组(A组,LLR长度<25.2 mm)和长LLR组(B组,LLR长度≥25.2 mm),每组40例,比较左心房CT三维重建相关指标和射频消融相关指标,分析射频消融术后房颤复发的预测因子。结果 A组左心房LLR长度短于B组,左心房容积低于B组(P<0.05或P<0.01)。A组手术时间、射频消融时间、左肺静脉射频消融时间均短于B组,平均接触压低于B组(P<0.05或P<0.01)。随访(471±104) d, A组射频消融成功率高于B组(87.5%vs. 55.0%)(P<0.01)。左心房LLR长度是射频消融术后房颤复发的独立预测因子(P<0.01)。结论 左心房LLR长度是射频消融术后房颤复发的独立预测因子。

Objective To explore the relationship between left lateral ridge(LLR) length of the left atrium(LA) and atrial fibrillation(AF) recurrence after radiofrequency ablation in the patients with AF.Methods Eighty patients with AF underwent three-dimensional CT reconstruction of the LA,who were divided into two groups of A(LLR length<25.2 mm) and B(LLR length≥25.2 mm). The relevant indexes of three-dimensional CT reconstruction of the LA and radiofrequency ablation were compared, and predictive factors for AF recurrence after radiofrequency ablation were analyzed.Results LLR length of LA was shorter and LA volume in group A was lower than those in group B(P<0.05 or P<0.01). The operation time, radiofrequency ablation time and left pulmonary vein ablation time were shorter, and average contact pressure of group A was lower than those of group B(P<0.05 or P<0.01). After followed-up for(471±104) days, the success rate of radiofrequency ablation in group A was higher than that in group B(87.5% vs. 55.0%)(P<0.01). LLR length of LA was an independent predictor of AF recurrence after radiofrequency ablation(P<0.01).Conclusion LLR length of LA is an independent predictor of AF recurrence after radiofrequency ablation.
