老年胸腰椎骨折患者术后骨缺损的影响因素

Influencing Factors of Postoperative Bone Defect in Elderly Patients with Thoracolumbar Fracture

目的 分析老年胸腰椎骨折患者术后骨缺损的影响因素。方法 选取2016年4月至2021年4月商丘市中医院收治的200例老年胸腰椎骨折患者作为研究对象,所有患者均接受后入路内固定融合术治疗,并于术后进行为期1 a随访,统计术后骨缺损发生情况,设计一般资料调查表,记录研究所需资料,采用logistic回归分析老年胸腰椎骨折患者术后骨缺损发生的影响因素。结果 发生组老年胸腰椎骨折患者骨折类型、椎间盘是否完整以及植骨情况与未发生组比较,差异有统计学意义(P<0.05);发生组老年胸腰椎骨折患者骨密度T值低于未发生组,差异有统计学意义(P<0.05)。logistic回归分析结果显示,压缩性骨折、椎间盘受损、骨密度T值低以及植骨情况是老年胸腰椎骨折患者术后骨缺损发生的影响因素(OR>1,P<0.05)。结论 压缩性骨折、椎间盘受损、骨密度T值低以及植骨情况是老年胸腰椎骨折患者术后骨缺损发生的影响因素。

Objective To explore the influencing factors of postoperative bone defect in elderly patients with thoracolumbar fractures. Methods A total of 200 elderly patients with thoracolumbar fractures treated in Shangqiu Hospital of Traditional Chinese Medicine from April 2016 to April 2021 were selected as the research subjects. All patients received posterior internal fixation and fusion and were followed up for 1 year after operation. The occurrence of postoperative bone defect was counted, the baseline data questionnaire was designed, and the data required for the study were recorded. Logistic regression was used to analyze the influencing factors of postoperative bone defect in elderly patients with thoracolumbar fracture. Results There were differences in the fracture type, integrity of intervertebral disc and bone grafting of elderly patients with thoracolumbar fracture between the occurrence group and the non occurrence group(P<0.05). The bone mineral density T value of elderly patients with thoracolumbar fracture in the occurrence group was lower than that in the non occurrence group, and the difference was statistically significant(P<0.05). Through logistic regression analysis, the results showed that compression fracture, intervertebral disc damage, low bone mineral density T value and bone grafting were the influencing factors of postoperative bone defect in elderly patients with thoracolumbar fractures(OR>1, P<0.05). Conclusion Compression fracture, intervertebral disc damage, low bone mineral density T value and bone grafting are the influencing factors of postoperative bone defect in elderly patients with thoracolumbar fractures.
