结肠癌患者内镜下黏膜切除术后自我感受负担的影响因素

Influencing Factors of Self-perceived Burden in Patients with Colorectal Cancer After Endoscopic Mucosal Resection

目的 分析结肠癌(CC)患者内镜下黏膜切除术(EMR)后自我感受负担(SPB)情况及相关影响因素。方法 选取南阳市第一人民医院2019年5月至2020年7月接受EMR治疗的83例CC患者研究对象,采用自我感受负担量表(SPBS)评估患者SPB情况,设计一般资料填写表,询问并记录患者相关资料,比较不同资料特征患者SPB情况,分析可能影响CC患者EMR术后SPB的相关因素。结果 83例CC患者EMR术后均伴有不同程度SPB,SPBS得分为33.00(17.00,42.00)分;自觉经济能力不足、家庭关怀一般、消极应对的CC患者SPBS评分均高于自觉经济能力充足、家庭关怀良好、积极应对的患者,差异有统计学意义(P<0.05);其他不同资料特征的CC患者SPBS评分对比,差异无统计学意义(P>0.05);多重线性回归分析结果显示,自觉经济能力不足、家庭关怀一般、消极应对均可能是CC患者EMR术后SPB严重的影响因素(P<0.05)。结论 CC患者EMR术后伴有不同程度SPB,自觉经济能力不足、家庭关怀一般、消极应对可能是影响CC患者EMR术后SPB的主要因素。

Objective To analyze the self-perceived burden(SPB) and related influencing factors in patients with colon cancer(CC) after endoscopic mucosal resection(EMR). Methods A total of 83 CC patients who received EMR treatment in Nanyang First People’s Hospital from May 2019 to July 2020 were selected as the research subjects. Self-perceived burden scale(SPBS) was used to evaluate the SPB of patients. The general data filling form was designed, and the related baseline information of patients was inquired and recorded. SPB of patients with different data characteristics were compared, and the related factors that might affect SPB after EMR in patients with CC were analyzed. Results A total of 83 patients with CC had different degrees of SPB after EMR, and the SPBS score was 33.00(17.00, 42.00) points. The SPBS scores of CC patients with self-awareness of insufficient economy, general family care and negative coping were higher than those with self-awareness of sufficient economy, good family care and positive coping, and the difference was statistically significant(P<0.05). There was no statistical significant difference in SPBS score among CC patients with other different characteristics(P>0.05). Multiple linear regression analysis showed that self-awareness of insufficient economy, general family care and negative coping might be the serious influencing factors of SPB in CC patients after EMR(P<0.05). Conclusion Patients with CC have different degrees of SPB after EMR, and the main influencing factors of SPB in patients with CC after EMR were self-awareness of insufficient economy, general family care and negative coping.
