血清miR-30a、miR-155水平与急性心肌梗死患者并发急性心力衰竭的相关性

摘要 目的 观察急性心肌梗死(AMI)患者血清微小核糖核酸-30a(miR-30a)、微小核糖核酸-155(miR-155)水平及急性心力衰竭并发情况,并分析二者与AMI患者并发急性心力衰竭的关系。方法 选取2019年8月至2021年7月收治的170例AMI患者,根据AMI患者入院48 h内急性心力衰竭发生情况分为心衰组与非心衰组,由研究者拟定临床资料调查表并进行资料统计,分析入院时血清miR-30a、miR-155水平与AMI患者并发急性心力衰竭的关系。结果 170例AMI患者并发急性心力衰竭42例,未并发急性心力衰竭128例;心衰组血清miR-30a、miR-155水平、梗死面积、肌酸激酶同工酶(CK-MB)、N-末端脑钠肽前体(NT-proBNP)水平高于非心衰组,左室射血分数低于非心衰组(P<0.05);经logistic回归分析,结果显示,血清miR-30a、miR-155、CK-MB以及NT-proBNP高表达、梗死面积大是AMI患者并发急性心力衰竭的危险因素(OR>1,P<0.05)。结论 入院时血清miR-30a、miR-155水平与AMI患者并发急性心力衰竭有关。 Objective To observe the expression of serum microRNA-30a(miR-30a), microRNA-155(miR-155) and the complication of acute heart failure in patients with acute myocardial infarction(AMI), and to analyze the relationship between them and AMI patients complicated with acute heart failure. Methods A total of 170 patients with AMI treated in the hospital from August 2019 to July 2021 were selected and divided into heart failure group and non heart failure group according to the occurrence of acute heart failure of patients within 48 hours after admission. The researchers drew up the clinical data questionnaire of the subjects to made data statistics, and analyzed the relationship between serum miR-30a, miR-155 and acute heart failure in patients with AMI. Results Among 170 patients with AMI, 42 cases were complicated with acute heart failure and 128 cases were not complicated with acute heart failure. The relative expression levels of serum miR-30a and miR-155, infarct size, creatine kinase isoenzymes-MB(CK-MB) and N-terminal pro-brain natriuretic peptide(NT-proBNP) in heart failure group were higher than those in non heart failure group, and the left ventricular ejection fraction was lower than that in non heart failure group(P<0.05). The results of logistic regression analysis showed that, the high expression of serum miR-30a, miR-155, CK-MB and NT-proBNP and large infarct size were the risk factors of AMI patients complicated with acute heart failure(OR>1, P<0.05). Conclusion The relative expression levels of serum miR-30a and miR-155 at admission are related to AMI patients complicated with acute heart failure.
