摘要
目的 探讨急诊重症加强护理病房(ICU)危重症伴失禁患者并发失禁相关性皮炎的高危因素。方法 选取2018年1月至2021年12月信阳市中心医院收治的500例急诊ICU危重症伴失禁患者作为本次研究对象,统计急诊ICU危重症伴失禁患者并发失禁相关性皮炎的情况。收集所有纳入患者的相关临床资料,采用logistic多元回归方程分析急诊ICU危重症伴失禁患者并发失禁相关性皮炎的高危因素。结果 500例急诊ICU危重症伴失禁患者中,并发失禁相关性皮炎40例;发生组与未发生组失禁频率、大便性状、压力性损伤危险(Braden)评分、意识障碍比较,差异有统计学意义(P<0.05);两组性别、年龄、体质量指数(BMI)、受教育程度、婚姻状况、职业类型、使用机械通气和镇静剂、抗生素种类比较,差异无统计学意义(P>0.05);经logistic回归分析可以得出,失禁频率、大便性状、Braden评分、意识障碍均为急诊ICU危重症伴失禁患者并发失禁相关性皮炎的高危因素(OR>1,P<0.05)。结论 急诊ICU危重症伴失禁患者并发失禁相关性皮炎的风险较高,失禁频率、大便性状、Braden评分、意识障碍是其发生的高危因素。
Objective To explore the high-risk factors of incontinence related dermatitis in critically ill patients with incontinence in emergency intensive care unit(ICU). Methods A total of 200 critically ill patients with incontinence in emergency ICU treated in Xinyang Central Hospital from January 2018 to December 2021 were selected as the study subjects. The status of incontinence related dermatitis in critically ill patients with incontinence in emergency ICU was analyzed. The relevant clinical data of all included patients were collected, and the high-risk factors of incontinence related dermatitis in critical patients with incontinence in emergency ICU were analyzed by logistic multiple regression equation. Results Among 500 critical patients with incontinence in emergency ICU, 40 cases had incontinence related dermatitis;compared between the occurrence group and the non occurrence group in incontinence frequency, stool character, pressure ulcer risk score(Braden), and consciousness disorder, the difference was statistically significant(P<0.05). There was no statistical significant difference between the two groups in gender, age, body mass index(BMI), educational level, marital status, occupational type, use of mechanical ventilation and sedatives, and types of antibiotics(P>0.05). Logistic regression analysis showed that incontinence frequency, stool characteristics, Braden score and disturbance of consciousness were all high-risk factors for incontinence related dermatitis in critically ill patients with incontinence in emergency ICU(OR>1, P<0.05). Conclusion The risk of incontinence related dermatitis in critically ill patients with incontinence in emergency ICU is higher, and incontinence frequency, stool characteristics, Braden score, and disturbance of consciousness are the high-risk factors.
