腹盆腔孤立性纤维性肿瘤的临床特征及手术疗效

Clinical Features and Surgical Outcomes of Solitary Fibrous Tumors in the Abdominal and Pelvic Cavity

目的探讨腹盆腔孤立性纤维性肿瘤(solitary fibrous tumor,SFT)的临床特点及手术疗效。方法回顾性分析2015年4月~2022年1月35例腹盆腔SFT手术的临床资料。17例无明显症状,12例腰腹部或骶尾部疼痛,4例下肢肿痛,2例反复低血糖。肿瘤中位最大径15 cm(3~25 cm)。初次手术22例,复发再手术13例。结果R0或R1切除32例,R2切除3例。联合脏器切除6例。7例行分块切除。中位术中出血量1200 ml(50~10000 ml)。术后并发症8例。随访3~68个月,中位数18个月。初次手术22例中4例复发,2例远处转移。结论手术完整切除是SFT最主要的治疗方法,术后易复发,需长期随访。

Objective To explore the clinical characteristics and treatment of solitary fibrous tumor(SFT)of abdominal and pelvic cavity.Methods Clinical data of 35 cases of abdominal and pelvic SFT surgery from April 2015 to January 2022 were retrospectively analyzed.Seventeen patients had no obvious symptoms,while 12 cases had waist,abdominal or sacrococcygeal pain,4 cases had lower limb swelling and pain,and 2 cases had recurrent hypoglycemia.The median length of the tumor was 15 cm(range,3-25 cm).There were 22 cases of primary surgery and 13 cases of reoperation for relapsed disease.Results There were 32 cases of R0 or R1 resection and 3 cases of R2 resection.There were 6 cases combined with organ resection.In 7 cases the resection was performed in blocks.The median intraoperative blood loss was 1200 ml(range,50-10000 ml).There were 8 cases of postoperative complications.Follow-ups lasted 3-68 months,with a median of 18 months.Among the 22 patients undergoing primary surgery,4 had recurrence and 2 had distant metastasis.Conclusion Complete surgical resection is the most important treatment for SFT,which is prone to recurrence after surgery and requires long-term follow-ups.
