血必净对尿源性脓毒症患者炎症反应及临床疗效的影响

Effect of Xuebijing on inflammatory response and clinical efficacy in patients with urosepsis

目的观察血必净对尿源性脓毒症患者的炎症反应及临床疗效的影响。方法回顾性分析2019年1月至2021年10月在本院急诊重症病房收治的85例尿源性脓毒症患者的临床资料,将患者分为对照组(40例)和血必净组(45例),对照组按照脓毒症指南采用综合治疗,血必净组为血必净联合综合治疗。比较治疗前及治疗7 d后两组的白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)、白介素-6(IL-6)、血小板计数(PLT)、尿素氮(BUN)、肌酐(CR)、发热天数及住院总天数。结果治疗前,两组的WBC、CRP、PCT、IL-6、PLT、BUN和CR比较,差异均无统计学意义(均P>0.05)。与治疗前比较,两组治疗7 d后的WBC、CRP、IL-6、BUN和CR均降低,且血必净组均低于对照组,差异均有统计学意义(均P<0.05)。与治疗前比较,两组治疗7 d后的PLT升高,且血必净组高于对照组(P<0.05)。血必净组的发热天数、住院总天数均短于对照组,差异均有统计学意义(均P<0.05)。结论血必净能够在一定程度上抑制尿源性脓毒症的炎症反应,加快肾功能的恢复,减少治疗时间。

Objective To observe the clinical effect of Xuebijing injection in the treatment urosepsis.Methods The clinical data of 85 patients with urogenic sepsis admitted to the emergency intensive care unit of our hospital from January 2019 to October 2021 were retrospectively analyzed,and the patients were divided into control group(40 cases)and Xuebijing group(45 cases).The control group received comprehensive treatment according to the sepsis guidelines,while the Xuebijing group received comprehensive treatment combined with Xuebijing.The levels of white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6),platelet count(PLT),urea nitrogen(BUN),creatinine(CR),days of fever and total days of hospitalization were compared before and 7 days after treatment.Results Before treatment,there were no significant differences in WBC,CRP,PCT,IL-6,PLT,BUN and CR between the two groups(all P>0.05).Compared with before treatment,WBC,CRP,PCT,IL-6,BUN and CR in two groups were decreased after 7 days of treatment,and Xuebijing group was lower than control group,the differences were statistically significant(all P<0.05).Compared with before treatment,the PLT of the two groups after 7 days of treatment was increased,and the PLT of the Xuebijing group was higher than that of the control group(P<0.05).Days of fever and total days of hospitalization in Xuebijing group were shorter than those in control group,and the differences were statistically significant(all P<0.05).Conclusions Xuebijing can inhibit the inflammatory response of urogenic sepsis to a certain extent,accelerate the recovery of renal function and reduce the treatment time.
