血清miR-135-5p及miR-337-5p在糖尿病肾病中的表达及其临床意义

Expression and clinical significance of serum miR-135-5p and miR-337-5p in diabetic nephropathy

目的探讨糖尿病肾病(DN)患者的血清miR-135-5p及miR-337-5p表达及其临床意义。方法选取2018年1月至2020年12月本院收治的105例2型DN患者和60例体检正常者(对照组)。根据尿白蛋白排泄率(UAER)将105例2型DN患者分为早期DN组57例(UAER为20~200μg/min)和临床期DN组48例(UAER>200μg/min)。比较各组血清miR-135-5p及miR-337-5p表达水平,应用多因素logistic回归分析影响DN发生的危险因素。绘制受试者工作特征(ROC)曲线分析血清miR-135-5p及miR-337-5p表达水平在诊断DN中的价值。结果DN组的血清miR-135-5p及miR-337-5p表达水平均明显高于对照组(均P<0.001)。临床期DN组的血清miR-135-5p及miR-337-5p表达水平均明显高于早期DN组(均P<0.001)。多因素logistic回归分析显示,血清miR-135-5p及miR-337-5p表达水平升高是影响DN发生的独立危险因素(均P<0.001)。ROC曲线分析显示,miR-135-5p及miR-337-5p两项联合诊断DN的曲线下面积最大(AUC=0.948,95%CI:0.887~0.995),其灵敏度为97.2%,特异度为85.0%。结论DN患者的血清miR-135-5p及miR-337-5p表达水平明显升高,两项联合检测对DN诊断具有较好的价值。

Objective To investigate the expression and clinical significance of serum miR-135-5p and miR-337-5p in patients with diabetic nephropathy(DN).Methods From January 2018 to December 2020,105 patients with DN and 60 patients with normal physical examination(control group)were selected.According to the ratio of urinary albumin to excretion rate(UAER),105 patients with DN were divided into early DN group 57 cases(UAER 20-200μg/min)and clinical DN group 48 cases(UAER>200μg/min).The expression levels of serum miR-135-5p and miR-337-5p in each group were compared,and the risk factors affecting the occurrence of DN were analyzed by multivariate logistic regression analysis.Receiver operating characteristic(ROC)curve was drawn to analyze the value of serum miR-135-5p and miR-337-5p expression level in the diagnosis of DN.Results The expression levels of serum miR-135-5p and miR-337-5p in DN group were significantly higher than those in control group(all P<0.001).The expression levels of miR-135-5p and miR-337-5p in clinical DN group were significantly higher than those in early DN group(all P<0.001).Multivariate logistic regression analysis showed that the increased expression levels of serum miR-135-5p and miR-337-5p were independent risk factors for DN(all P<0.001).ROC curve showed that the area Tunder the curve(AUC=0.948,95%CI:0.887-0.995)of miR-135-5p and miR-337-5p in the joint diagnosis of DN was the largest,and their sensitivity and specificity were 97.2%and 85.0%.Conclusions The expression levels of serum miR-135-5p and miR-337-5p in patients with DN are significantly increased,and the combined detection of the two items has good value in the diagnosis of DN.
