甲状腺功能减退患者孕前调控与孕后治疗的临床效果对比

Comparison of clinical effects between pre-pregnancy regulation and post-pregnancy treatment in patients with hypothyroidism

目的对比甲状腺功能减退患者孕前调控与孕后治疗的临床效果。方法选取2018年6月至2021年6月在济宁市妇幼保健计划生育服务中心参加孕前检查的115例甲状腺功能减退备孕患者(孕前组)及113例甲状腺功能减退合并妊娠≥6周患者(孕后组)为研究对象。比较两组妊娠合并症及并发症发生情况、妊娠结局及围产儿情况。结果孕前组合并症及并发症总发生率和不良妊娠结局总发生率均低于孕后组,且孕前组围产儿情况优于孕后组,差异有统计学意义(P<0.05)。结论孕前调控甲状腺功能对妊娠结局具有积极意义,临床治疗妊娠合并甲状腺功能减退应定期监测甲状腺激素水平变化。

Objective To compare the clinical effects of pre-pregnancy regulation and post-pregnancy treatment in patients with hypothyroidism.Methods A total of 115 patients with hypothyroidism preparing for pregnancy(prepregnancy group)and 113 patients with hypothyroidism complicating pregnancy≥6 weeks(post-pregnancy group)who participated in pre-pregnancy examination in Jining Maternal and Child Health Family Planning Service Center from June 2018 to June 2021 were selected as the research objects.The pregnancy complications,pregnancy outcomes and perinatal condition were compared between the two groups.Results The total incidence of complications and adverse pregnancy outcomes in the pre-pregnancy group was lower than that in the post-pregnancy group,the perinatal condition of the pre-pregnancy group was better than that of the post-pregnancy group,and the differences were statistically significant(P<0.05).Conclusion The regulation of thyroid function before pregnancy has positive significance to the outcome of pregnancy.The change of thyroid hormone level should be monitored regularly in clinical treatment of pregnancy complicated with hypothyroidism.
