内镜下肾上腺素注射、氩气刀电凝联合钛夹治疗十二指肠溃疡出血的效果分析

Analysis on the Effect of Endoscopic Epinephrine Injection,Argon Knife Electrocoagulation Combined with Titanium Clip in the Treatment of Duodenal Ulcer Bleeding

目的分析内镜下肾上腺素注射、氩气刀电凝联合钛夹治疗十二指肠溃疡出血的效果。方法选取我院2018年1月至2020年12月收治的110例十二指肠溃疡出血患者,按照治疗方式不同分为观察组(n=58)与对照组(n=52)。观察组行内镜下肾上腺素注射、氩气刀电凝联合钛夹治疗,对照组行内镜下肾上腺素注射联合氩气刀电凝治疗。比较两组患者的止血效果、止血时间、出血量、住院时间及凝血功能指标(PT、Fib)。结果两组患者的止血总有效率及治疗后PT、Fib水平比较差异无统计学意义(P>0.05)。观察组的止血时间、住院时间短于对照组,出血量少于对照组(P<0.05)。结论内镜下肾上腺素注射、氩气刀电凝联合钛夹治疗十二指肠溃疡出血的疗效与内镜下肾上腺素注射联合氩气刀电凝治疗相当,但可显著缩短止血时间,减少出血量,促进患者快速康复。

Objective To analyze the effect of endoscopic adrenaline injection,argon knife electrocoagulation combined with titanium clip in the treatment of duodenal ulcer bleeding.Methods 110 cases of patients with duodenal ulcer hemorrhage admitted to our hospital from January 2018 to December 2020 were selected and divided into observation group(n=58)and control group(n=52)according to different treatment methods.The observation group was treated with endoscopic epinephrine injection,argon knife electrocoagulation combined with titanium clip,and the control group was treated with endoscopic epinephrine injection combined with argon knife electrocoagulation.The hemostasis effect,hemostasis time,blood loss,hospitalization time and coagulation function indicators(PT,Fib)were compared between the two groups.Results No statistical difference was found in the total effective rate of hemostasis,and the PT and Fib levels after treatment were compared between the two groups(P>0.05).The hemostasis time and hospitalization time of the observation group were shorter than those of the control group,and the blood loss was less than that of the control group(P<0.05).Conclusions Endoscopic adrenaline injection,argon knife electrocoagulation combined with titanium clip in the treatment of duodenal ulcer bleeding has similar efficacy with endoscopic epinephrine injection combined with argon knife electrocoagulation,but can significantly shorten the hemostasis time,reduce the blood loss,and promote the rapid recovery of patients.
