Epidemiological characteristics of stage-promotion cases of occupational pneumoconiosis in Taiyuan from 2006 to 2020

摘要 目的通过对2006-2020年太原市职业性尘肺病晋期病例流行病学特征分析,为卫生行政部门制定职业性尘肺病的防治对策提供一定的数据参考。方法从中国疾病预防控制信息系统职业病及健康危害因素监测信息系统中导出的经三级审核通过的卡片,采用SPSS 21.0统计软件进行分析。结果2006-2020年,太原市共报告284例尘肺病晋期病例319例次。职业性煤工尘肺晋期病例219例,占77.1%;矽肺61例,占21.5%;电焊工尘肺2例,占0.7%;铸工尘肺2例,占0.7%。职业性尘肺壹期晋贰期242例次,占75.9%,贰期晋叁期57例次,占17.9%,壹期晋叁期20例次,占6.2%。性别分布上,以男性晋期病例为主,共282例,占99.3%。主要分布在万柏林区195例,占68.6%,其次为古交市49例,占17.3%,尖草坪区30例,占10.6%。企业规模上,主要分布在19家大型企业,共259例,占91.2%。壹期晋叁期晋期年限最长(16.2 a),壹期晋贰期晋期年限最短(5.0 a)。壹期晋贰期病例中,铸工尘肺晋期年限最长,为14.5 a,其次为矽肺(10.0 a),煤工尘肺(4.1 a),电焊工尘肺晋期年限最短(3.1 a);贰期晋叁期病例中,矽肺晋期年限中位数长于煤工尘肺晋期年限5.0 a;壹期晋叁期病例中,矽肺晋期年限中位数长于煤工尘肺晋期年限6.1 a。结论太原市职业性尘肺病晋期病例主要以大、中型企业的壹期晋贰期煤工尘肺为主,晋期年限中位数为4.1 a。 Objective To analyze the epidemiological characteristics of stage-promotion cases reported in Taiyuan City from 2006 to 2020,providing certain data reference to the health administration departments to formulate prevention and treatment policies for occupational pneumoconiosis.Methods Three-level audited cards have been analyzed by SPSS 21.0 statistical software,which are obtained from the Occupational Diseases and Health Hazard Factors Monitoring Information System of China Information System for Disease Control and Prevention.Results A total of 284 stage-promotion cases of occupational pneumoconiosis and 319 times of stage-promotion were reported in Taiyuan City from 2006 to 2020.There were 219 coal worker’s pneumoconiosis(77.1%),61 silicosis(21.5%),2 welder’s pneumoconiosis(0.7%)and 2 founder pneumoconiosis(0.7%).There were 242 cases in the first stage promoting to the second stage(75.9%),57 cases in the second stage promoting to the third stage(17.9%)and 20 cases in the first stage promoting to the third stage(6.2%).In terms of gender distribution,282 cases(99.3%)were male.There were 195 cases(68.6%)distributed in Wanbailin District,followed by 49 cases(17.3%)in Gujiao City and 30 cases(10.6%)in Jiancaoping District.In terms of enterprises scale,259 cases(91.2%)were distributed in 19 large enterprises.It took the longest time to promote from the first stage to the third stage(16.2 a).The first stage promoting to the second period took the shortest time(5.0 a).Among the cases with promotion from first stage to the second stage,founder pneumoconiosis had the longest time of promotion(14.5 a),followed by silicosis(10.0 a),coal worker’s pneumoconiosis(4.1 a),and welder’s pneumoconiosis has the shortest time of promoting stage(3.1 a).Among cases with promotion from the second stage to the third stage,the median length of promotion in silicosis was longer than that in coal workers’pneumoconiosis,Among cases with promotion from the first stage to the third stage,the median length of promotion in silicosis was longer than that in coal workers’pneumoconiosis.Conclusions The stage-promotion cases of occupational pneumoconiosis in Taiyuan City were mainly coal workers’pneumoconiosis from large and medium-sized enterprises,and most of them promoted from the first stage to the second stage,and the length of promoting stage was 4.1 a.
