带导向及固定装置双腔CVC胸腔闭式引流术用于行胸腔镜肺叶切除术的NSCLC患者的效果研究

Effect of Double Chamber CVC Closed Thoracic Drainage with Guidance and Fixation Device for Patients with NSCLC Undergoing Thoracoscopic Lobectomy

目的:探讨带导向及固定装置双腔中心静脉导管(CVC)胸腔闭式引流术应用于行胸腔镜肺叶切除术非小细胞肺癌(NSCLC)患者的效果。方法:选取2021年1月-2022年2月在承德县医院接受胸腔镜肺叶切除术治疗的62例NSCLC患者,通过随机数字表法分为两组,各31例。围手术期给予对照组传统术后引流干预,观察组则行带导向及固定装置的双腔CVC胸腔闭式引流干预。观察两组围手术期相关指标、术后疼痛程度、睡眠质量、不良反应及满意度等情况。结果:观察组术中出血量、住院费用少于对照组,术后ICU停留时间、引流管留置时间及术后住院时间均短于对照组,差异均有统计学意义(P<0.05);术后24、48、72 h,观察组的视觉模拟评分法(VAS)、匹兹堡睡眠质量评价表(PSQI)评分均低于对照组,差异有统计学意义(P<0.05);两组术后并发症总发生率比较,差异无统计学意义(P>0.05);观察组的总满意度高于对照组,差异有统计学意义(P<0.05)。结论:双腔CVC胸腔闭式引流技术应用于胸腔镜手术NSCLC患者,能够有效改善围手术期相关指标,减轻患者术后疼痛程度,改善患者睡眠质量,获得更高满意度。

Objective:To investigate the effect of double chamber central venous catheter(CVC)closed thoracic drainage with guidance and fixation device for patients with non-small cell lung cancer(NSCLC)undergoing thoracoscopic lobectomy.Method:A total of 62 NSCLC patients who received thoracoscopic lobectomy in Chengde County Hospital from January 2021 to February 2022 were selected and divided into two groups by random number table method,with 31 cases in each group.During perioperative period,the control group was given traditional postoperative drainage intervention,while the observation group was given double chamber CVC closed thoracic drainage with guidance and fixation device intervention.The perioperative related indexes,postoperative pain,sleep quality,adverse reactions and satisfaction were observed in the two groups.Result:The amount of intraoperative bleeding and hospitalization expenses in the observation group were less than those in the control group,and the stay time in ICU,drainage tube retention time and postoperative hospital stay in the observation group were shorter than those in the control group(P<0.05).At 24,48,72 hours after operation,the visual analogue scale(VAS)and Pittsburgh sleep quality index(PSQI)scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).The total satisfaction rate in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Double chamber CVC closed thoracic drainage with guidance and fixation device used in thoracoscopic NSCLC patients can effectively improve the perioperative index,reduce the postoperative pain degree,improve their sleep quality,and obtain higher satisfaction.
