超声介入聚桂醇栓塞治疗颌面部血管瘤的临床效果及安全性评价

Clinical Efficacy and Safety Evaluation of Ultrasonic Interventional Lauromacrogol Embolization in Maxillofacial Hemangioma

目的:分析超声介入聚桂醇栓塞治疗颌面部血管瘤的临床效果及安全性。方法:选择天水市第一人民医院2019年1月-2021年6月接诊的48例颌面部血管瘤患者作为研究对象,随机分为对照组和观察组,各24例。对照组予以瘤腔内注射平阳霉素治疗,观察组予以超声介入聚桂醇栓塞治疗。随访6个月,检测两组不同时间点的瘤体面积,记录治疗相关并发症发生情况,比较两组皮肤颜色消退、瘤体缩小和整体效果的满意度评分及临床总有效率。结果:治疗前,两组瘤体面积比较,差异无统计学意义(P>0.05);治疗后30、90、180 d,观察组瘤体面积均明显小于对照组,差异均有统计学意义(P<0.05)。两组均未见皮肤坏死、过敏性休克等严重治疗相关并发症,其他并发症总发生率比较,差异无统计学意义(P>0.05)。观察组皮肤颜色消退、瘤体缩小和整体效果的满意度评分均高于对照组,差异均有统计学意义(P<0.05)。观察组临床总有效率为95.83%,高于对照组的75.00%,差异有统计学意义(P<0.05)。结论:超声介入聚桂醇栓塞治疗颌面部血管瘤的效果好,起效快,安全可靠且患者满意度高,值得临床予以重视应用。

Objective:To analyze the clinical effect and safety of ultrasonic interventional Lauromacrogol embolization for maxillofacial hemangioma.Method:A total of 48 maxillofacial hemangioma patients admitted in Tianshui First People’s Hospital from January 2019 to June 2021 were selected as the research object,and they were randomly divided into control group and observation group,with 24 cases in each group.The control group was treated with intratumoral injection of Bleomycin,and the observation group was treated with ultrasonic interventional Lauromacrogol embolization.Followed up for 6 months,measured the tumor area at different time points of the two groups,recorded the occurrence of treatment related complications,and compared the satisfaction scores of skin color fading,tumor shrinking and overall effect and the total clinical efficiency of the two groups.Result:Before treatment,there was no significant difference in tumor area between the two groups(P>0.05).At 30,90,180 d after treatment,the tumor area in observation group were significantly smaller than those in control group,the differences were statistically significant(P<0.05).No severe treatment-related complications such as skin necrosis and anaphylactic shock were found between the two groups,and there was no significant difference in the total incidence of other complications(P>0.05).The satisfaction scores of skin color regression,tumor shrinkage and overall effect in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total clinical effective rate in observation group was 95.83%,which was higher than 75.00%in control group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasonic interventional Lauromacrogol embolization for maxillofacial hemangioma is effective,safe,reliable and highly satisfactory,which deserves clinical attention.
