散光矫正型双焦点人工晶状体治疗年龄相关性白内障的效果及对术后双眼视觉质量的影响

Efficacy of Astigmatism Corrected Bifocal Intraocular Lens in the Treatment of Age-related Cataract and the Influence on Binocular Visual Quality

目的:探究不同附加度数散光矫正型双焦点人工晶状体(IOL)治疗年龄相关性白内障(ARC)的效果及对术后双眼视觉质量的影响。方法:回顾性分析钟祥市人民医院2020年3月-2021年3月收治的ARC患者,根据散光矫正型双焦点IOL不同附加度数分为A组(n=22,44眼,分别植入+2.5 D和+3.0 D散光矫正型双焦点IOL)和B组(n=26,52眼,均植入+3.0 D散光矫正型双焦点IOL)。比较两组单眼裸眼视力、单眼矫正视力、术后双眼视觉质量及术后并发症。结果:术后3个月,两组裸眼远视力(UCDVA)、裸眼近视力(UCNVA)、最佳矫正远视力(BCDVA)、最佳矫正近视力(BCNVA)比较,差异无统计学意义(P>0.05);但A组裸眼中视力(UCMVA)、最佳矫正中视力(BCMVA)优于B组(P<0.05)。术后3个月,A组视功能量表(NEI-VFQ-25)评分中整体视力水平、近距离活动评分高于B组(P<0.05)。两组术后重影,异物感、光晕等不良反应比较,差异无统计学意义(P>0.05)。结论:双眼分别植入不同附加度数的散光矫正型双焦点IOL可以提高全程视力和术后视觉质量。

Objective:To investigate the effect of different degrees of astigmatic corrected bifocal intraocular lens(IOL)in the treatment of age related cataract(ARC)and its influence on binocular visual quality after surgery.Method:Retrospective analysis was made on 48 patients with ARC admitted to Zhongxiang People’s Hospital from March 2020 to March 2021.They were divided into group A(n=22,44 eyes,implanted with+2.5 D and+3.0 D astigmatic bifocal IOL respectively)and group B(n=26,52 eyes,implanted with+3.0 D astigmatic bifocal IOL respectively)according to different additional degrees of astigmatic bifocal IOL.The uncorrected visual acuity,corrected visual acuity,binocular visual quality and postoperative complications were compared between the two groups.Result:Three months after operation,there were no significant differences between the two groups in the uncorrected distance visual acuity(UCDVA),uncorrected near visual acuity(UCNVA),best corrected distance visual acuity(BCDVA),best corrected near visual acuity(BCNVA)(P>0.05).However,the uncorrected medium visual acuity(UCMVA)and best corrected medium visual acuity(BCMVA)in group A were better than those in group B(P<0.05).Three months after operation,the overall visual acuity level and the score of close range activity in the National Eye Institute 25-item visual function questionnaire(NEI-VFQ-25)score of group A were higher than those in group B(P<0.05).There were no significant differences between the two groups in terms of adverse reactions such as ghost,foreign body sensation and halo(P>0.05).Conclusion:Implantation of astigmatic corrected bifocal IOL with different additional degrees in both eyes can improve the overall visual acuity and postoperative visual quality.
