3D-STI评价不同分期尘肺病患者右心室功能的价值

Value of 3D-STI in Evaluating Right Ventricular Function in Patients with Pneumoconiosis at Different Stages

目的探讨三维斑点追踪(3D-STI)技术评价不同分期尘肺病患者右心室功能的价值。方法回顾性分析2019年1月-2021年5月于本院就诊的104例尘肺病患者,将其分为Ⅰ期尘肺病49例及Ⅱ期尘肺病55例,所有患者均接受3D-STI检查,且病历资料显示图像清晰,观察两期患者右心室基底段及中段检查结果。结果Ⅰ期尘肺病患者GRS、GCS、PASP水平均低于Ⅱ期尘肺病患者,GLS、RVEF、TVSPV、FAC值均高于Ⅱ期尘肺病患者(P<0.05);Ⅰ期尘肺病患者基底段CS、RS、LS值均高于Ⅱ期尘肺病患者,中段CS、LS值均高于Ⅱ期尘肺病患者,RS值低于Ⅱ期尘肺病患者(P<0.05)。结论3D-STI可根据不同分期尘肺病患者右心室圆周、径向、纵向及血磷状态改变情况较为客观、全面的判断右心室功能。

Objective To explore the value of three-dimensional speckle tracking(3D-STI)in evaluating right ventricular function in patients with pneumoconiosis at different stages.Methods 104 patients with pneumoconiosis treated in our hospital from January 2019 to may 2021 were analyzed retrospectively.They were divided into 49 cases of stage I pneumoconiosis and 55 cases of stageⅡpneumoconiosis.All patients underwent 3D-STI,and the medical records showed clear images.The examination results of the basal and middle segments of the right ventricle were observed.Results The levels of GRS,GCS and PASP in patients with stage I pneumoconiosis were lower than those in patients with stageⅡpneumoconiosis,and the values of GLS,RVEF,tvspv and FAC were higher than those in patients with stageⅡpneumoconiosis(P<0.05);The values of CS,RS and LS in the basal segment of patients with stage I pneumoconiosis were higher than those in patients with stageⅡpneumoconiosis,the values of CS and LS in the middle segment were higher than those in patients with stageⅡpneumoconiosis,and the values of RS were lower than those in patients with stageⅡpneumoconiosis(P<0.05).Conclusion 3D-STI can objectively and comprehensively judge the right ventricular function according to the changes of right ventricular circumference,radial,longitudinal and blood phosphorus status in patients with pneumoconiosis at different stages.
