全程跟踪护理对于维持性血液透析患者血管通路再循环率及并发症发生率的作用探讨

Effect Discussion of Whole Process Follow-Up Nursing on Vascular Access Recirculation Rate and Complication Incidence of Maintenance Hemodialysis Patients

目的探究全程跟踪护理在维持性血液透析(MHD)患者中的应用效果。方法选择2018年3月-2020年3月在本院进行MHD治疗的200例患者,其中100例患者实施全程跟踪护理(观察组),另100例患者则采取常规护理(对照组),对比其护理效果,即UN再循环率及Scr再循环率、并发症发生率、护理的满意率以及活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)、纤维蛋白原(Fbg)凝血指标水平。结果经过护理后,观察组患者的BUN再循环率及Scr再循环率均明显小于对照组差异有统计学意义(P<0.05);观察组并发症发生率(28.0%)明显低于对照组(32.0%),差异有统计学意义(P<0.05);此外,观察组对全程跟踪护理的满意率(95.0%)明显高于对照组对常规护理的满意度(86.0%),差异有统计学意义(P<0.05);凝血指标水平比较,干预前两组患者的aPTT、PT、Fbg接近(P>0.05)。干预后两组患者的aPTT、PT提升,Fbg下降,且观察组指标提升与下降幅度大于对照组(P<0.05)。结论在MHD患者的治疗过程中实施全程跟踪护理模式,可显著改善患者的凝血功能指标,进而提升患者的治疗安全性及护理满意度,具有显著的应用价值。

Objective To explore application effect of whole process follow-up nursing for maintenance hemodialysis(MHD)patients.Methods The paper chose 200 patients treated with MHD in our hospital from March 2018 to March 2020.Among them,100 cases were treated with whole process follow-up nursing(observation group),and the other 100 cases with routine nursing(control group).Nursing effect was compared between groups,namely,UN recycling rate and Scr recycling rate,complication rate,nursing satisfaction degree,activated partial thromboplastin time(aPTT),prothrombin time(PT)Fibrinogen(Fbg)coagulation index level.Results After nursing,BUN recycling rate and Scr recycling rate in observation group was significantly lower than control group(P<0.05);complications incidence in observation group(28.0%)was significantly lower than control group(32.0%),with statistically significant difference(P<0.05);in addition,satisfaction degree of observation group(95.0%)with whole process follow-up nursing was significantly higher than control group(86.0%),with statistically significant difference(P<0.05);coagulation index level comparison showed,aPTT,PT and Fbg between two groups were close to before intervention(P>0.05).After intervention,aPTT and PT of two groups increased,Fbg decreased,indexes increased and decreased in observation group were more than control group(P<0.05).Conclusion Whole process tracking nursing mode can improve the coagulation function indicators significantly in treatment of MHD patients,and enhance their treatment safety and nursing satisfaction degree,with significant application value.
