基于OCTA检测高度近视患者视网膜和脉络膜微血流改变的META分析 被引量:1

摘要 目的:系统评价OCTA检测高度近视患者视网膜及脉络膜毛细血管密度、视神经纤维层的改变。方法:检索PubMed、中国知网及万方数据库查找相关已发表的研究。主要观察黄斑区浅层及深层微血管密度、脉络膜血管密度及视网膜神经纤维层厚度、中心凹无血管区面积(FAZ)。结果:纳入2017年后9篇符合条件的文献进行Meta分析。高度近视组黄斑区视网膜浅、深血管密度及脉络膜毛细血管密度低于对照组,高度近视眼视神经纤维层厚度也明显降低。高度近视组黄斑区无血管区面积无明显改变。结论:在近视进程中,视网膜和脉络膜血管网密度的改变可能与眼轴伸长有关,高度近视患者微血管网的改变是病理性近视的发病基础,OCTA是一种有效的、无创的近视进展监测手段。 objective:To systematically evaluate the changes of retinal and choroid capillary density and optic nerve fiber layer detected by OCTA in patients with high myopia.Methods:PubMed,CNKI and Wanfang databases were searched for relevant published studies.The density of superficial and deep microvessels in macular area,the density of choroidal vessels,the thickness of retinal nerve fiber layer,and the area of fovea without blood vessels(FAZ)were mainly observed.Results:Nine eligible literatures after 2017 were included for meta-analysis.The density of superficial and deep retinal blood vessels and choroidal capillary vessels in the macular area in the high myopia group were lower than those in the control group,and the thickness of optic nerve fiber layer in the high myopia group was also significantly reduced.There was no significant change in the area of vascularized macular area in the high myopia group.Conclusion:In the course of myopia,changes in retinal and choroidal vascular network density may be related to axial elongation of the eye.Changes in the microvascular network in patients with high myopia are the basis of pathological myopia.OCTA is an effective and non-invasive means to monitor the progression of myopia.
