HCV-RNA和抗HCV在丙肝病毒感染检测中的临床应用

摘要 目的探讨HCV-RNA和抗HCV在丙肝病毒感染检测中的临床应用。方法以2019年6月-2020年11月在本院就诊的门诊及住院患者496例为研究对象,取患者血清样本,使用实时荧光定量聚合酶链反应(qPCR)检测患者的感染情况,固相时间分辨免疫荧光分析法(TRIFMA)检测样本中抗HCV(包括IgG、IgM)的含量,分析不同检测方法的诊断效能并进行统计分析。结果对496份样品进行检测,HCV-RNA结果显示阳性73例,对HCV感染患者的检测敏感性为88.75%,特异性为99.52%;抗HCV结果显示阳性96例,阴性400例,对HCV感染患者的检测敏感性为90.00%,特异性为94.23%;二者联合检测阳性122例,对HCV感染患者的检测敏感性为97.50%,特异性为89.42%;HCV-RNA和抗HCV联合检测的敏感性高于HCV-RNA检测,特异性低于HCV-RNA检测,差异均有统计学意义(P<0.05);抗HCV检测的特异性低于HCV-RNA检测,差异有统计学意义(P<0.05);80例HCV感染患者中,HCV-RNA和抗HCV检出情况间的差异有统计学意义(P<0.05)。结论HCV-RNA检测特异性高于抗HCV检测,二者联合检测能够显著提升敏感性,联合检查上述两种指标有利于丙肝病毒流行的防治。 Objective To probe into the clinical application of HCV-RNA and anti HCV in detection of hepatitis C virus infection.Methods 496 cases of outpatients and inpatients and who treated in our hospital from June 2019 to November 2020 were selected as the research objects,and the serum samples were collected from patients,the infection of the patients were detected by the real time fluorescent quantitative polymerase chain reaction(qPCR),and the content of anti HCV(including IgG and IgM)in the samples were detected by the solid-phase time-resolved immunofluorescence assay(TRIFMA).Then,the diagnostic efficiency of different detection methods was counted and statistically analyzed.Results 496 samples were tested,73 were positive by HCV-RNA.The sensitivity and specificity of detection of HCV infection in patients were 88.75%and 99.52%,respectively;anti HCV results showed that 96 cases were positive and 400 cases were negative.The sensitivity and specificity of detection of HCV infection were 90.00%and 94.23%respectively;the sensitivity of combined detection of HCV-RNA and anti HCV was higher than that of HCV-RNA,and the specificity was lower than that of HCV-RNA,and the differences were statistically significant(P<0.05);the specificity of anti HCV detection was lower than that of HCV-RNA detection,and the difference was statistically significant(P<0.05);in 80 patients with HCV infection,the difference between HCV-RNA and anti HCV detection was statistically significant(P<0.05).Conclusion The specificity of HCV-RNA detection is higher than that of anti HCV detection.The combined detection of HCV-RNA and anti HCV can significantly improve the sensitivity,and the combined prevention and treatment of hepatitis C virus is beneficial to the above two epidemic indicators.
