2型糖尿病患者糖基化终末产物水平的临床影响因素研究

Clinical Factors Related to Advanced Glycation End Products in Patients with Type 2 Diabetes Mellitus

目的分析2型糖尿病患者糖基化终末产物(Advanced glycation end products,AGEs)累积量与各临床影响因素的相关性,为2型糖尿病的中医诊疗及其相关并发症的早期防治提供参考。方法选取2018年12月—2020年12月期间于开封市中医院内分泌科住院的2型糖尿病患者285例,收集其AGEs、实验室检查及中医证候信息。采用描述性统计分析,非参数检验、斯皮尔曼相关性分析,最优尺度回归方法分析AGEs的相关影响因素。结果AGEs水平与果糖胺(Glycosylated serum protein,GSP)、糖化血红蛋白等实验室指标呈现相关关系,在不同中医证型中AGEs水平存在显著的差异。中医证候、年龄、病程、尿微量白蛋白是2型糖尿病患者AGEs水平的独立影响因素。结论AGEs水平与临床影响因素具有相关性,为2型糖尿病的中医诊疗及其并发症的防治提供了参考。

Objective To analyze the correlation between the accumulation of advanced glycation end products(AGEs)and clinical factors in patients with type 2 diabetes mellitus(T2 DM),and provide references for traditional Chinese medicine(TCM)diagnosis and treatment of T2 DM and early prevention and treatment of related complications.Methods A total of 285 patients with T2 DM hospitalized at Kaifeng Hospital of Traditional Chinese Medicine from December 2018 to December 2020 were included.The information on AGEs,laboratory examination,and TCM syndrome scores of the patients were collected.The correlation factors of AGEs were analyzed by descriptive statistical analysis,non-parametric test,Spearman correlation analysis,and optimal scale regression.Results The levels of AGEs were correlated with glycosylated serum protein(GSP),glycosylated hemoglobin(HbA1 c),and other laboratory indicators,and there were significant differences in the levels of AGEs in different TCM syndromes.TCM syndromes,age,course of the disease,and urinary microalbumin(mALB)were independent influencing factors of the levels of AGEs in patients with T2 DM.Conclusion The levels of AGEs were correlated with clinical influencing factors,which provide references for TCM diagnosis and treatment of T2 DM and the prevention and treatment of its complications.
