超声造影对直径≤3 cm小肝癌分化程度的诊断价值

Diagnostic value of contrast-enhanced ultrasonography in differentiation of small hepatocellular carcinoma with diameter≤3 cm

目的探讨超声造影对直径≤3 cm小肝癌分化程度的诊断价值。方法选取经肝穿刺活检/病理检查确诊为小肝癌的患者89例(共96个病灶)。对比不同分化程度直径≤3 cm小肝癌患者的超声造影定量参数水平、回声增强模式及与病理检查结果的符合率。结果89例小肝癌患者(共96个病灶)病灶直径均≤3 cm,其中13个病灶为低分化,32个病灶为中分化,51个病灶为高分化。不同分化程度直径≤3 cm小肝癌患者消退时间比较,差异有统计学意义(P<0.01)。在门脉相和延迟相中,不同分化程度直径≤3 cm小肝癌患者的回声增强模式比较,差异均有统计学意义(P<0.01)。超声造影诊断≤3 cm小肝癌分化程度的结果与病理结果的符合率为88.54%(85/96)。结论超声造影诊断直径≤3 cm小肝癌分化程度具有重要价值,可为其临床诊断和后续治疗提供可靠资料。

Objective To analyze the diagnostic value of contrast-enhanced ultrasonography in differentiation of small hepatocellular carcinoma with diameter≤3 cm.Method A total of 89 patients with small hepatocellular carcinoma(96 lesions)confirmed by transhepatic biopsy/surgical pathological examination were selected.The quantitative parameters of contrast-enhanced ultrasonography,the echo-enhancement pattern and the coincidence rate with the pathological examination of small hepatocellular carcinoma with diameter≤3 cm in different degrees of differentiation were compared.Result The diameters of lesions were all≤3 cm in 89 patients with small hepatocellular carcinoma(96 lesions),of which13 lesions were poorly differentiated,32 were moderately differentiated,and 51 were well differentiated.There was significant difference for the regression time of small hepatocellular carcinoma with diameter≤3 cm with different differentiation degrees(P<0.01).In portal vein phase and delayed phase,there were significant differences for echo-enhancement patterns of small hepatocellular carcinoma with diameter≤3 cm with different differentiation degrees(P<0.01).The coincidence rate of contrast-enhanced ultrasonography with pathological results in diagnosing differentiation degree of small hepatocellular carcinoma with diameter≤3 cm was 88.54%(85/96).Conclusion Contrast-enhanced ultrasonography are of great value in evaluating the differentiation of small hepatocellular carcinoma with diameter≤3 cm,which can provide reliable data for clinical diagnosis and subsequent treatment.
