丹蒌通痹汤治疗痰瘀阻络型冠心病心绞痛临床观察

Clinical Study of Danlou Tongbi Decoction on Angina Pectoris of Coronary Heart Disease with Phlegm and Blood Stasis

目的观察丹蒌通痹汤治疗痰瘀阻络型冠心病心绞痛临床疗效。方法将60例入组患者按照随机分组原则分为试验组与对照组各30例。对照组给予冠心病常规西医基础治疗,试验组在西医治疗基础上合用丹蒌通痹汤,治疗周期均为4周。观察两组治疗前后中医证候疗效、心绞痛发作次数、疼痛持续时间、疼痛程度、心电图疗效及血脂[低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)]水平。结果对照组有1例患者主动要求退出研究,最终试验组纳入30例(男性16例,女性14例),对照组纳入29例(男性13例,女性16例)。对照组中医证候疗效总有效率为86.67%,高于对照组的65.52%(P<0.05)。治疗后试验组心绞痛发作次数、疼痛持续时间、疼痛程度、LDL-C及TC水平均低于对照组(均P<0.05)。试验组心电图总有效率80.00%高于对照组的55.17%(P<0.05)。两组治疗前后三大常规、肝肾功能均未出现异常。结论丹蒌通痹汤治疗痰瘀阻络型冠心病心绞痛可有效改善患者症状,减少心绞痛发作次数、疼痛持续时间、疼痛程度,有效降低LDL-C水平,提高患者生活质量。

Objective:To observe the clinical efficacy of Danlou Tongbi Decoction on angina pectoris of coronary heart disease with phlegm and blood stasis.Methods:A total of 60 patients were randomly divided into the experimental group(30 cases)and control group(30 cases).The control group was given conventional western medicine treatment for coronary heart disease,and the experimental group was given Danlou Tongbi Decoction on the basis of western medicine treatment.The treatment course was 4 weeks.The curative effect on TCM syndrome,angina attack times,pain duration,pain degree,electrocardiogram effect and the reduction level of low density lipoprotein(LDL-C)and total cholesterol(TC)were observed in the two groups.Results:One patient in the control group requested to withdraw from the study.Finally,30 patients(16 males and 14 females)were enrolled in the experimental group and 29 patients(13 males and 16 females)in the control group.The total effective rate of TCM syndrome in the control group was 86.67%,which was higher than 65.52%in the control group(P<0.05).After treatment,the incidence of angina pectoris,pain duration,pain degree,LDL-C and TC in the experimental group were lower than those in the control group(all P<0.05).The total effective rate of the experimental group was80.00%,which was higher than 55.17%of the control group(P<0.05).Before and after treatment,there were no abnormalities in the three major routines,liver and kidney functions in the two groups.Conclusions:Danlou Tongbi Decoction can effectively improve the symptoms of patients with angina pectoris of phlegm-stasis type coronary heart disease,reduce the frequency of angina pectoris,pain duration,pain degree,effectively reduce the level of LDL-C,improve the quality of life of patients,which is worthy of clinical application.
