摘要
目的 评估扩散加权成像(DWI)联合动态增强MR成像定量指标用于乳腺癌患者术前新辅助化疗有效性中的价值。方法 选取2019年7月—2020年2月该院收治的43例乳腺癌患者为研究对象,对所有患者均行扩散加权成像(DWI)联合动态增强MR成像检查,记录MRI中的定量指标(LD、ADC、E、Emax),并分析各个指标的变化率以及对病理反应的诊断效能。结果 二组患者LD值比较差异无统计学意义(P>0.05);但MHR组ADC、E、Emax值则均高于NMHR组(P<0.05);以术后病理反应为“金标准”,绘制ROC曲线图,各参数在乳腺癌中具有较好的诊断效能;二元Logistic回归分析显示,LD、ADC、E、Emax与乳腺癌患者术前辅助化疗反应性呈正相关性,表明其与乳腺癌患者术前新辅助化疗有效性存在密切关联。结论 扩散加权成像(DWI)联合动态增强MR成像定量指标有助于评估乳腺癌患者术前辅助化疗疗效,值得临床借鉴。
Objective To study and explore the value of diffusion weighted imaging(DWI)combined with dynamic contrast enhanced MR imaging in evaluating the effectiveness of preoperative neoadjuvant chemotherapy in patients with breast cance.Methods Select forty-three patients with breast cancer treated in our hospital from July 2019 to February 2020 as research objects.The weighted imaging(DWI)joint dynamic enhancement MR imaging examination was performed for all patients.Record the quantitative index of MRI scan(LD,ADC,E,Emax.,Record and analyze the change rate of each parameter and the diagnostic effectiveness of the pathological reaction.ResultsThere is no significant difference in the LD value of the two groups of patients(P>0.05);but the MHR group ADC,E,and EMAX values are higher than the NMHR group(P<0.05);The picture,each parameter has a good diagnosis in breast cancer;the regression analysis of the dual logistic regression shows that LD,ADC,E,EMAX and breast cancer patients are positive There is a close relationship with the reactivity of the front auxiliary chemotherapy.Conclusion Diversifying the weighted imaging(DWI)joint dynamic enhancement MR imaging quantitative index can help evaluate the efficacy of pre-surgery chemotherapy for patients with breast cancer,which is worthy of clinical reference.
