文拉法辛联合托吡酯治疗偏头痛伴广泛性焦虑障碍患者的效果

Effects of Venlafaxine combined with Topiramate in treatment of migraine patients with generalized anxiety disorder

目的:观察文拉法辛联合托吡酯治疗偏头痛伴广泛性焦虑障碍患者的效果。方法:回顾性分析2021年1—11月该院收治的86例偏头痛伴广泛性焦虑障碍患者的临床资料,按照治疗方法不同将其分为观察组44例、对照组42例。两组均予以托吡酯治疗,在此基础上,对照组予以奥沙西泮治疗,观察组予以文拉法辛治疗。比较两组临床疗效、治疗前后头痛[视觉模拟评分法(VAS)]评分、对生活的影响程度[头痛影响测定-6(HIT-6)]评分、焦虑程度[汉密尔顿焦虑量表(HAMA)]评分、睡眠质量[匹兹堡睡眠质量指数(PSQI)]评分和不良反应发生率。结果:治疗后,观察组治疗总有效率为95.45%(42/44),高于对照组的78.57%(33/42),差异有统计学意义(P<0.05);观察组VAS、HIT-6、HAMA、PSQI评分均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:文拉法辛联合托吡酯治疗偏头痛伴广泛性焦虑障碍患者可提高治疗效果,降低头痛及其影响程度评分,缓解焦虑情绪,改善睡眠质量。

Objective: To observe effects of Venlafaxine combined with Topiramate in treatment of migraine patients with generalized anxiety disorder. Methods: The clinical data of 86 migraine patients with generalized anxiety disorder admitted to this hospital from January to November 2021 were retrospectively analyzed. According to different treatment methods, they were divided into observation group(44 cases)and control group(42 cases). Both groups were treated with Topiramate. On this basis, the control group was treated with Oxazepam, while the observation group was treated with Venlafaxine. The clinical efficacy, the headache [visual analogue scale(VAS)] score, the influence degree [headache impact test-6(HIT-6)] score, the anxiety degree [Hamilton anxiety scale(HAMA)] score, the sleep quality [Pittsburgh sleep quality index(PSQI)]score, and the incidence of adverse reactions were compared between the two groups. Results: After the treatment, the total effective rate of the observation group was 95.45%(42/44), which was higher than 78.57%(33/42) of the control group, and the difference was statistically significant(P<0.05). The scores of VAS, HIT-6, HAMA and PSQI in the observation group were lower than those in the control group, and the differences were statistically significant(P<0.05).However, there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusions: Venlafaxine combined with Topiramate in the treatment of the migraine patients with generalized anxiety disorder can improve the total effective rate of treatment and reduce the VAS, HIT-6, HAMA and PSQI scores. Moreover, it is superior to Oxazepam combined with Topiramate treatment.
