黄体酮胶囊联合屈螺酮炔雌醇片治疗围绝经期功能失调性子宫出血患者的效果

Effects of progesterone capsules combined with Drospirenone and Ethinylestradiol tablets in treatment of perimenopausal dysfunctional uterine bleeding

目的:观察黄体酮胶囊联合屈螺酮炔雌醇片治疗围绝经期功能失调性子宫出血(功血)患者的效果。方法:回顾性分析2018年1月至2021年3月该院收治的62例围绝经期功血患者的临床资料,按照治疗方法不同将其分为观察组和对照组各31例。对照组予以屈螺酮炔雌醇片治疗,观察组在对照组基础上联合黄体酮胶囊治疗。比较两组临床疗效、治疗前后子宫内膜厚度、月经量[月经失血图(PBAC)]、性激素指标[卵泡刺激素(FSH)、黄体生成素(LH)和雌二醇(E_(2))]、血红蛋白(Hb)、血细胞比容(Hct)水平,以及不良反应发生率。结果:观察组治疗总有效率为93.55%(29/31),高于对照组的61.29%(19/31),差异有统计学意义(P<0.05);治疗后,观察组子宫内膜厚度和PBAC评分均低于对照组,差异有统计学意义(P<0.05);观察组E_(2)、Hb、Hct水平均高于对照组,FSH、LH水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:黄体酮胶囊联合屈螺酮炔雌醇片治疗围绝经期功血患者可提高治疗总有效率和Hb、Hct水平,改善性激素指标水平,以及降低子宫内膜厚度和PBAC评分,效果优于单纯屈螺酮炔雌醇片治疗。

Objective: To observe effects of progesterone capsules combined with Drospirenone and Ethinylestradiol tablets in treatment of perimenopausal dysfunctional uterine bleeding(hereinafter referred to as "dysfunctional uterine bleeding"). Methods: The clinical data of 62 patients with perimenopausal dysfunctional uterine bleeding admitted to this hospital from January 2018 to March 2021 were retrospectively analyzed.According to different treatment methods, they were divided into observation group and control group, 31 cases in each. The control group was treated with Drospirenone and Ethinylestradiol tablets, while the observation group was treated with Progesterone capsules on the basis of that of the control group. The clinical efficacy, the endometrial thickness before and after the treatment, the menstrual volume [menstrual blood loss assessment chart(PBAC)], the sex hormone index levels [follicle stimulating hormone(FSH), luteinizing hormone(LH) and estradiol(E_(2))], the hemoglobin(Hb)and hematocrit(Hct) levels, and the incidence of adverse reactions were compared between the two groups. Results: The total effective rate of the observation group was 93.55%(29/31), which was higher than 61.29%(19/31) of the control group, and the difference was statistically significant(P<0.05). After the treatment, the endometrial thickness and the PBAC score of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05). The levels of E_(2), Hb and Hct in the observation group were higher than those in the control group, the levels of FSH and LH were lower than those in the control group, and the differences were statistically significant(P<0.05). However, there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusions: Progesterone capsules combined with Drospirenone and Ethinylestradiol tablets in the treatment of perimenopausal dysfunctional uterine bleeding can improve the total effective rate of treatment and the levels of Hb and Hct, improve the levels of sex hormones, and reduce the endometrial thickness and the PBAC score. Moreover,it is superior to single Drospirenone and Ethinylestradiol tablets.
