联勤保障部队第960医院近十年溃疡性结肠炎肠外表现的临床分析

Summary and analysis on extraintestinal manifestations in ulcerative colitis of No.960 Hospital of the recent decade of years

目的总结和分析联勤保障部队第960医院近十年溃疡性结肠炎(ulcerative colitis,UC)的肠外表现,提高对UC伴发肠外表现的诊断和治疗能力,为临床诊断提供思路与方法。方法本研究纳入了2011年1月1日至2020年12月31日在联勤保障部队第960医院就诊并住院的UC患者,选取其中具有肠外表现的资料进行分析。结果共对459例UC病例进行分析,男278例(60.57%),女181例(39.43%),男∶女为1.54∶1;发病年龄(42.92±14.95)岁(10~78岁),其中高发人群为20~59岁的中青年人,以40~49岁患者最多;2011-2015年UC病例数316例,2016-2020年病例数143例;459例UC患者中有355例(77.34%)伴有肠外表现,男219例,女136例,比例为1.61∶1;其中119例(25.93%)伴有一种肠外表现,236例(51.42%)伴有两种及以上肠外表现。出现肠外表现者第二季度最多,为116例,第一季度为88例,第三季度为70例,第四季度为81例。结论UC常伴有多种肠外表现,除累及结肠外,其肠外表现可同时累及多个系统,导致多系统性病变。本研究中其发生率较高,为UC患者的3/4,好发于中青年,以凝血功能异常及肝胆胰病变最为多见。

Objective To summary and analysis the extraintestinal manifestations in ulcerative colitis(UC),and improve its effects of diagnosis and treatment,then provide the methods for the clinic.Methods The clinical data of 459 patients with UC from Jan.1st,2011 to Dec.31st,2020 were retrospectively analyzed.And the extraintestinal manifestations were selected.Results A total of 459 reports of UC were included.The sexes of the patients were 278 males(60.6%)and 181 females(39.4%).The ratio of males to females was 1.54∶1.The average ages were(42.92±14.95)years old;a high-risk group was 20~59 years old,which the most was 40~49 years old.There were 316 patients from 2011 to 2015,and 143 patients from 2016 to 2020.355 patients(77.34%)combined with extraintestinal manifestations.The sexes of the patients were 219 males and 136 females.The ratio of males to females was 1.61∶1;119 patients(25.93%)had one extraintestinal manifestation,236 patients(51.42%)had more than one extraintestinal manifestation.88 patients had extraintestinal manifestations in the first quarter;116 patients in the second quarter;70 patients in the third quarter;81 patients in the forth quarter.Conclusion UC is often accompanied by a variety of extraintestinal manifestations.In addition to involving the colon,its extraintestinal manifestations can involve multiple systems at the same time,resulting in multi-system lesions.This study showed that the incidence of UC is high,about 3/4 of the total number of UC patients.It occurs in young and middle-aged people,with coagulation dysfunction and hepatobiliary and pancreatic lesions.
