质量管理工具在不良事件改进中的实施效果评价

The implementation and effectiveness evaluation of comprehensive application of quality management tools in analysis and improvement of adverse events

目的探讨合理选择、联合应用质量管理工具在处理医学检验科医疗不良事件,持续改进检验质量中的应用效果。方法根据不良事件的严重程度确定不良事件处理流程,通过判断不良事件的性质选择质量管理工具,单一或联合应用根本原因分析法、PDCA、质量指标等质量管理工具对医学检验科发生的不良事件进行查因整改,分析典型案例中质量管理工具的应用过程及效果。结果选择及联合应用质量管理工具对不良事件进行改进的过程中,不良事件得到有效解决,落实了全程质量管理,提升了工作人员能力,关键质量指标如危急值报告率达到100%,历次室间质评及格率分别达到97%、100%、98%、98%,提高了检验质量,促进了同行共同进步。结论质量管理工具的合理综合应用有利于提高不良事件的改进效果,达到提高检验质量的目的。

Objective To explore the effects of quality management tools reasonably selected and jointly applied in handling medical adverse events in the department of laboratory medicine and continuously improving inspection quality.Methods The adverse event handling process was determined on the basis of the severity of adverse events.Besides,quality management tools were selected by judging the nature of adverse events.Root Cause Analysis,PDCA,quality indicators and other quality management tools were singly or jointly applied to identify the reason for and rectify adverse events in the department of laboratory medicine and analyze the application process and effects of quality management tools in typical cases.Results During the improvement of adverse events by the reasonably selected and jointly applied quality management tools,adverse events were effectively resolved;the whole-process quality management was implemented;the capacities of the staff were improved;the report rate of key quality indicators such as critical value reached 100%and the pass rate of all external quality assessments reached 97%,100%,98%and 98%,respectively,improving the inspection quality and promoting the common progress of peers.Conclusion The reasonable comprehensive application of quality management tools is beneficial to improve adverse events and achieve the purpose of improving the inspection quality.
