细节控制对减少先天性心脏病体外循环手术后患儿肺部感染的作用分析

Analysis of the Role of Detail Control in Reducing Pulmonary Infections in Children after Extracorporeal Circula⁃tion Surgery for Congenital Heart Disease

目的:探究细节控制对减少先天性心脏病体外循环手术后患儿肺部感染影响性,以期提高疗效,降低感染。方法:选取2012年5月—2013年5月河南科技大学第一附属医院收治的50例先天性心脏病体外循环手术患儿作为研究对象,随机分成两组,每组各25例,对照组常规治疗,观察组在对照组基础上细节控制,观察治疗后在相关指标变化情况。结果:对照组肺部感染发生率为36%,观察组肺部感染率为16%,组间比较差异有统计学意义(χ^(2)=8.478,P<0.05);观察组在肺部湿啰音消失时间、咳嗽消失时间、住院时间上较对照组明显缩短,两组患儿比较差异有统计学意义(t=6.363、7.123、6.893,P<0.05);治疗后,观察组白细胞、C反应蛋白、血沉较对照组治疗后比较,差异有统计学意义(t=6.267、7.258、8.123,P<0.05)。结论:细节控制能减少先天性心脏病体外循环手术后肺部感染发生率,缩短治疗时间。

Objective:To investigate the impact of detail control on reducing pulmonary infections in children after extracorpo⁃real circulation surgery for congenital heart disease,with a view to improving outcomes and reducing infections.Methods:50 chil⁃dren admitted to the hospital from May 2012 to May 2013 with extracorporeal circulation surgery for congenital heart disease were selected as study subjects and randomly divided into two groups,with 25 cases in each group.The control group was treated rou⁃tinely,and the observation group was controlled in detail on the basis of the control group.After treatment in the relevant index changes were observed.Results:The incidence of pulmonary infection was 36%in the control group and 16%in the observation group,with a statistically significant difference(χ^(2)=8.478,P<0.05).The observation group had significantly shorter time to disap⁃pearance of wet rales in the lungs,time to disappearance of cough,and hospital stay than the control group,and the differences were statistically significant(t=6.363,7.123,6.893,P<0.05).After treatment,there were statistically significant differences in leu⁃kocytes,C-reactive protein and blood sedimentation in the observation group compared with the control group(t=6.267,7.258,8.123,P<0.05).Conclusion:Detail control reduces the incidence of pulmonary infections after extracorporeal circulation surgery for congenital heart disease and shortens the duration of treatment.
