老年患者前庭性偏头痛与脑卒中的相关研究

Research on the correlation between vestibular migraine and stroke in the elderly

目的本研究前瞻性分析老年患者前庭性偏头痛(VM)与脑卒中的相关性。方法连续纳入2020年12月至2021年12月佛山市禅城区永安医院神经内科住院的首次发病的急性脑卒中患者共计200例,将同时诊断为脑卒中和VM的患者归为VM组(59例),不能诊断为VM的脑卒中患者归为NVM组(141例)。采用logistic回归分析老年患者VM与脑卒中的关系。结果VM组冠心病、吸烟比例及收缩压水平明显高于NVM组,VM组年龄小于NVM组(P<0.05)。两组TOAST分型构成比比较,差异无统计学意义(P>0.05)。调整年龄、冠心病、吸烟史、收缩压水平后,多因素logistic回归分析显示,VM、年龄是脑卒中的危险因素(P<0.05)。单因素回归分析显示,老年患者VM是脑卒中的危险因素(P<0.05)。结论老年患者VM与脑卒中相关。除性别、年龄因素外,VM也是脑卒中的独立危险因素。

Objective To prospectively analyze the correlation between vestibular migraine(VM)and stroke in elderly patients.Methods A total of 200 patients with acute stroke who were hospitalized for the first time in the Department of Neurology,Chancheng District Yong'an Hospital of Foshan from December 2020 to December 2021 were continuously included,and they were divided into the VM group(n=59)and the NVM group(n=141).The patients who were diagnosed as stroke and VM at the same time were classified as the VM group,while the patients with stroke who could not be diagnosed as VM were classified as the NVM group.Logistic regression was adopted to analyze the correlation between VM and stroke in elderly patients.Results The proportion of coronary heart disease and smoking and systolic blood pressure level in the VM group were significantly higher than those in the NVM group,and the patients in the VM group were younger than those in the NVM group(P<0.05).There was no statistically significant difference in the constituent ratio of TOAST typing between the two groups(P>0.05).After adjusting age,coronary heart disease,smoking history and systolic blood pressure level,multivariate logistic regression analysis showed that VM and age were risk factors for stroke(P<0.05).Univariate regression analysis showed that VM was a risk factor for stroke in elderly patients(P<0.05).Conclusion There is a correlation between VM and stroke in the elderly.Besides gender and age factors,VM is also an independent risk factor for stroke.
