温州地区精神病专科医院住院患者院内感染现状的多中心调查

A multicenter survey for nosocomial infection in patients of psychiatric hospitals in Wenzhou

目的了解温州地区精神病专科医院住院患者的院内感染现状。方法采取分层抽样法,使用自制调查表调查2019年3月~2021年3月温州地区6所精神病专科医院3866例住院患者的院内感染现状。结果温州地区精神病专科医院总体院内感染发生率为2.9%,其中私立医院的院内感染发生率高于公立医院(P=0.004)。感染部位以呼吸道(62.2%)最为常见,其次为泌尿道和胃肠道;院内感染患者的抗菌药物使用率高达88.3%,给药途径以口服为主,占80.2%,用药方式主要是单一用药,占75.7%。多因素回归分析结果提示,高龄(>60岁)长时间住院(≥7天)以及糖尿病是院内感染的独立风险因素(均P<0.05)。结论高龄、长时间住院和糖尿病是精神病专科医院患者院内感染的风险因素,应以此为依据加强院内感染防控。

Objective To identify the current status of nosocomial infections in patients admitted to the psychiatric hospitals in Wenzhou.Methods This study enrolled 3866 inpatients from 6 psychiatric hospitals in Wenzhou during March 2019 to March 2021.The prevalence rate and independent associated factors for nosocomial infections were analyzed.Results The overall incidence of nosocomial infections in psychiatric hospitals in Wenzhou was 2.9%,with the incidence of nosocomial infections in private hospitals being higher than that in public hospitals(P<0.05).the respiratory tract(62.2%)was the most common site of infection,fllowed by the gastrointestinal tract and urinary system infection;the rate of antibacterial drug use in patients with nosocomial infections was as high as 88.3%,and the route of administration was mainly oral(80.2%),and the mode of administration of was mainly single drug(75.7).The results of multivariate analysis suggested that advanced age(>60 years old),prolonged hospitalization(≥7 days)and diabetes mellitus were risk factors for nosocomial infections.Conclusion Advanced age,prolonged hospitalization and diabetes mellitus were risk factors for nosocomial infections in patients in psychiatric hospitals,and should be used as a basis for strengthening nosocomial infections prevent and control.
