某院静脉药物超药品说明书用药的帕累托图分析

Off-Label Intravenous Drug Use in a Hospital Based on Pareto Chart Analysis

目的促进静脉药物的临床合理使用。方法采用回顾性分析法,抽取医院信息系统2021年7月至12月的所有住院患者的静脉用药长期医嘱,以药品说明书及相关文献为依据,判断静脉药物是否超药品说明书用药。采用帕累托图分析法分析超药品说明书用药的类型和药物种类。结果共收集医嘱1236372条,纳入有效医嘱18549条(1.50%),其中超药品说明书用药医嘱676条(3.64%)。超药品说明书用药类型主要因素为超剂量(26.33%)、超适应证(21.30%)、超给药频次(17.31%)和超溶剂(13.61%);药物种类主要因素为呼吸系统药物(42.01%)和消化系统药物(24.85%)。结论审方药师可根据超药品说明书用药的具体情况有针对性地进行干预,以促进静脉药物的临床合理使用。

Objective To promote the clinical rational use of intravenous drugs.Methods The long-term medical orders of intravenous drug use of all inpatients in the hospital information system from July to December 2021 were extracted and retrospectively analyzed.The drug instructions and relevant studies were as the reference to judge whether the use of intravenous drugs was off-label drug use.The Pareto chart analysis method was used to analyze the types of off-label drug use and related drug categories.Results A total of 1236372 medical orders were collected,and 18549 effective medical orders(1.50%)were included,in which 676 medical orders(3.64%)had the problem of off-label drug use.The main types of off-label drug use were drug overdose(26.33%),inappropriate indications(21.30%),inappropriate frequency of use(17.31%)and menstruum mistake(13.61%).The main drug categories were respiratory system drugs(42.01%)and digestive system drugs(24.85%).Conclusion Prescription-reviewing pharmacists can provide effective interventions according to the specific off-label drug use to promote the clinical rational use of intravenous drugs.
