摘要
目的:研究精神分裂症遗传高危人群失匹配负波及时频特征,探索精神分裂症遗传风险识别的客观指标。方法:采用横断面设计,纳入精神分裂症患者(SCZ)、精神分裂症遗传高危人群(FHR)及健康对照(HC)各19例。采用精神病风险综合征结构式访谈(SIPS)、阳性和阴性症状量表(PANSS)及阴性症状评估量表-16(NSA-16)评估精神症状严重程度,采用精神分裂症认知功能简短评估(BACS)评估神经认知功能。利用Oddball范式采集脑电信号,比较持续时间失匹配负波(dMMN)振幅,theta诱发功率及相位一致性(ITC)。结果:SCZ的NSA-16得分高于FHR及HC(P<0.001),BACS得分低于FHR及HC(P<0.001)。SCZ的dMMN振幅低于FHR及HC(P<0.05),theta诱发功率低于FHR及HC(P<0.05)。三组间ITC差异存在趋势(P=0.078)。结论:本研究未发现精神分裂症遗传高危人群与健康人群相比存在dMMN及theta振荡异常。
Objective:To study mismatched negativity and time-frequency characteristics in people at familial high risk for schizophrenia to explore objective indicators of genetic risk.Method:In this cross-sectional study,19 patients with schizophrenia,19 subjects at familial high-risk for schizophrenia(FHR group),and 19 normal controls were included.The Structured Interview for Prodromal Symptoms(SIPS),Positive and Negative Syndrome Scale(PANSS)and Negative Symptom Scale-16(NSA-16)were used to assess the severity of symptoms.The Brief Assessment of Cognition in Schizophrenia(BACS)were used to assess neurocognitive function.The electroencephalogram signals were collected by using oddball paradigm,and the amplitude of duration mismatched negativity(dMMN),evoked power and inter-trial coherence(ITC)in theta-band were compared between the groups.Result:The NSA-16 scores were higher and the BACS scores were lower in the patients with schizophrenia than in the FHR group and normal controls(Ps<0.001).The dMMN amplitude and theta-band evoked power were lower in the patients with schizophrenia than in the FHR group and normal controls(Ps<0.05).ITC difference between groups were at trend level(P=0.078).Conclusion:In this study,no abnormal mismatched negativity and theta oscillation were found in people at familial high risk for schizophrenia compared with the normal population.
