桃红四物汤加味联合左卡尼汀对维持性血液透析患者残余肾功能及血沉作用的研究

Effects of Modified Taohong Siwu Decoction combined with L-carnitine on residual renal function and erythrocyte sedimentation rate in maintenance hemodialysis patients

目的观察桃红四物汤加味联合左卡尼汀治疗维持性血液透析(MHD)患者的疗效、对残余肾功能和血沉的作用。方法将我院70例MHD患者随机分为观察组与对照组各35例。对照组采用左卡尼汀等常规西医方案治疗,观察组在对照组治疗基础上给予桃红四物汤加味治疗,比较两组患者临床疗效,治疗前后残余肾功能(RRF)、血肌酐(Scr)、血尿素氮(BUN)、血红蛋白(Hb)、血白蛋白(ALB)、前白蛋白(PA)、血沉水平。结果治疗后,观察组患者总有效率高于对照组(P<0.05);两组患者RRF、Scr、BUN水平与治疗前比较均下降,且观察组患者RRF水平高于对照组,Scr、BUN水平低于对照组(P<0.05);两组患者Hb水平与治疗前比较增加,观察组患者ALB、PA水平与治疗前比较均增加,对照组患者ALB、PA水平与治疗前比较均减少,且观察组患者Hb、ALB、PA水平高于对照组(P<0.05);两组患者血沉较治疗前减少,且观察组患者血沉低于对照组(P<0.05)。结论对于MHD患者,采用桃红四物汤加味及左卡尼汀治疗显著提高效果,延缓残余肾功能降低,降低血沉,调节机体营养状态。

Objective To observe the efficacy of Modified Taohong Siwu Decoction combined with L-carnitine in treatment of maintenance hemodialysis(MHD),and its effect on residual renal function and erythrocyte sedimentation rate.Methods Seventy MHD patients were randomly divided into observation group and control group,35 cases in each.Control group was treated with conventional western medicine such as L-carnitine,and observation group was treated with Modified Taohong Siwu Decoction on the basis of control group.The residual renal function(RRF),serum creatinine(Scr)and blood urea nitrogen(BUN),hemoglobin(Hb),serum albumin(ALB),prealbumin(PA),erythrocyte sedimentation rate(ESR)before and after treatment were compared between the two groups;the curative effect of the two groups was calculated.Results After treatment,the total effective rate of observation group was higher than that of control group(P<0.05);the RRF,Scr and BUN levels decreased compared with before treatment in both groups,and the RRF level of observation group were higher,and the Scr and BUN levels were lower than in control group(P<0.05);the Hb levels increased compared with before treatment in both groups,the ALB and PA levels in observation group increased compared with before treatment,the ALB and PA levels decreased in control group compared with before treatment,and the Hb,ALB,and PA levels in observation group were higher than those in control group(P<0.05);the ESR reduced in both groups compared with before treatment,and was in lower observation group than in control group(P<0.05).Conclusion Modified Taohong Siwu decoction combined with L-carnitine significantly improves the effect,delays the reduction of residual renal function,reduces ESR,and regulates the nutritional status of the body in treatment of MHD.
